Alleviating long and unnecessary stays in the hospital, our home infusion team administer intravenous (IV) therapy and treatment in a safe and comfortable setting. Through our experienced and professional pharmacy team, AmeriMed assists patients recovering from surgery, being treated for an infection, or with nutritional needs to maintain an active, healthy lifestyle while receiving care at home. Our goal is to help each patient be an active participant in their care. Through education and partnership with each patient and their caregivers, we teach our patients how to effectively manage their therapy and provide ongoing oversight, coordination and support throughout their therapy.

AmeriMed Home Infusion in Indianapolis, IN is looking for a Care Transition Nurse to join our team!

Responsibilities

The Care Transition Nurse (CTN) acts as a field-based business development representative and is responsible for growing the market and meeting the expectations for referrals and admissions. They observe and report trends, changes, and new opportunities in the market and identify ways to increase awareness, educate and promote home infusion services.

The Care Transition Nurse focuses on cultivating relationships with referral sources which include area hospitals, rehab facilities, nursing homes and physicians. Helping to educate them on the variety of services offered by our company, increasing awareness on the benefits of home care, and identifying how and when services are appropriate for individuals and would provide an optimal solution for the patients needs.

The Care Transition Nurse is a general home care resource and provides value added counsel, teaching and resourceful problem solving to the patient, health care provider and facilities. The CTN aids in the referral process by determining eligibility, explaining Medicare & Medicaid guidelines, explaining insurance plan benefits, financing options, obtaining verbal or written orders, communicating with appropriate disciplines involved in the care, and providing education to the patient to assist patients in making a seamless transition to home infusion services.

Qualifications

To be considered for this amazing opportunity, candidate should possess the following:

• Be a graduate of an accredited school of nursing, with a current RN or LPN license in Indiana*
• Current CPR Certification; with ability to perform CPR.*
• Have a minimum of Two years clinical nursing experience. Hospital or home care nursing experience is preferred. Home Infusion experience is a major plus, but not required.*
• Knowledge of home care regulations is required.
• Results oriented professional, self-motivated exhibiting strong initiative, vitality, and follow through.
• Excellent communication skills. Ability to engage, connect, and tailor message to audience. Effective communication is key to success in this role.
• Exhibits professionalism in daily tasks and ability to build strong relationships and trust with referral sources.
• Must be a critical thinker, with good judgment, creativity and decision making skills.

*In substation for post high school level education and nursing experience candidates must have five years IV/Home Medical Equipment customer service experience and a Medical Terminology certification to be considered.
