MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

You will be responsible for promoting the benefit of utilization of our Centers’ approved days for our participants. You will have a direct impact in enhancing the health and well-being of our participants and increasing the utilization of Connected Care’s services.

What You'll Do


  • Deliver calls & communication to participants, their responsible parties, and payers regarding the benefits of utilization of approved days
  • Establish an opportunity in each communication to optimize the contact or connection to each participant
  • Provide excellent participant service in all calls and communication
  • Maintain strong knowledge of Centers’ programming and unique or special features
  • Assist in recording and sharing potential Center quality issues (as shared by Participant, stakeholder, or payer) with Program Director or supervisor
  • Model professionalism in all contacts and exhibit good judgment related to private or sensitive information
  • Follows up communications as necessary and keeps the participant informed of the status
  • Participate in coaching, debriefing, and strategy sessions with supervisor and team
  • Provide assistance with enrollment process as needed; facilitating with completion of necessary next steps

What You’ll Need

  • Excellent sales/telephonic skills with proven track record (preferred)
  • Listens attentively and ensures understanding of what is being said
  • Ability to persuade, solve problems, and overcome objections and obstacles
  • Ability to be independent and autonomous, while also open to collaboration, feedback, and suggestions in a marketing/sales team-based environment
  • Ability to track results, prioritize efforts, and develop reporting mechanisms, all by balancing multiple responsibilities
  • Organizes work tasks to maximize quality and completeness
  • Proficient in Microsoft Office – Excel, Word, Outlook
  • Bilingual – English/Spanish – highly preferred
  • Must be 18 years of age or older
  • High School graduate or equivalent (GED)
  • Eligibility to work in the US without Company sponsorship for a work visa or employment-based green card

Additional Requirements While Working Remote


  • Must have an internet connection with a minimum of 10 mbps download and 5 mbps upload speed from a reliable internet provider (Cable, DSL, Satellite or Fiber Optic/FiOS providers).
  • Must be able to work in HIPAA compliant environment at home and abide by all HIPAA best practices
  • Must reside in MA/NH or CT- Position may require limited travel to Foxborough, MA
