Nutrition Coordinator

Fremont Area

Overall Summary

Prevent and treat illness by promoting healthy eating habits and recommending dietary modifications. Provide nutritional services by identifying patients’ individual needs and assisting with , implementing and evaluating nutrition programs. Educate patients on nutritional practices designed to prevent disease and promote healthy eating. Instruct patients and their families in nutritional care plans to include diet modification, grocery shopping and food preparation. Assist RD’s with group nutrition class for chronic and pediatric patients. Work with medical providers and other care providers to coordinate medical and nutritional needs. Provide continuity of care by scheduling appointments with RD’s for 1:1 counseling and recruit for nutrition classes.


Essential Responsibilities

  • Manage Nutrition referrals and schedule appointments
  • Coordinate the nutrition services at the clinic level and with community programs.
  • Provide nutrition support and education to high-risk patients.
  • Assist, coordinate, and lead group nutrition class for chronic and pediatric patients.
  • Present oral and written ideas in a clear, concise and persuasive manner to professionals and the public. Nutrition Services Coordinators often are called upon to provide information on the program or nutrition to co-workers and other community partners.
  • Assist in planning process for nutrition services and procure, print, and create nutrition education materials for patients.
  • Instruct patients and/or their families regarding diets; develop and provide instructional materials to assist patients in their home dietary program.
  • Evaluate effectiveness of dietary care plan in relation to patient's medical progress
  • Assist to outreach patients with chronic diseases such as obesity, hypertension and diabetes.
  • Food Farmacy Events: Plan and coordinate meal distribution and items to distributed, order produce, coordinate flyers, and all items needed for Food Farmacy.
  • Work flexible schedule as necessary. Some Saturdays required for outreach events.
  • Travel to other locations as needed.
  • Lifting up to 50lbs. Bending, lifting, pushing and pulling required for this position.


QUALIFICATIONS

Required education, experience, training

  • Prior experience in, health education, food and nutrition, or a related field.
  • Bilingual – Spanish highly desirable
  • Experience in providing direct nutrition services to families, mothers, and children


Preferred education, experience, training

  • 6 months-1 year of directly related experience in Nutrition.
  • Experience in public health or community health.
  • Associates degree Preferred.


Certifications/Licenses:

  • Health Educator (Desired)
