CARE COORDINATOR I - CAROLINA PATIENT SUPPORT SERVICES
This position supports the Mission, Values, and Vision of Carolina Family Health Centers, Inc. The Care Coordinator I will assist the medical providers in providing comprehensive medical care to patients through care coordination and population health.
Essential Responsibilities:
Make contact with patients who have been identified as being lost to care to help bring them back into care and help with the productivity of providers.
Close the referral loop by obtaining radiology and specialty reports/office visit notes, marking the referral closed in the referral work queue, and entering results in the electronic medical record.

Run monthly reports to identify patients with open care gaps, make contact with the patients to educate them on the need for such service, and notify the medical provider.

Track and provide outreach to prenatal care patients until the time of delivery.

Assist with quality reporting and auditing.

Contact patients prior (or at the time of their visit) to their annual wellness visits to conduct a health risk assessment, remind the patient of their upcoming appointment and prepare printed materials for the provider.

Provide risk assessment for identified patient populations which will include an assessment of social determinants of health.

Assist medical providers in the completion of forms for insurance prior authorizations and medical supplies.

Minimum Qualifications:
  • High school diploma or GED.
  • Certified Nursing Assistant.
  • Valid NC Driver License.
  • BLS certification.
  • Basic computer skills.
  • Must be detail and team oriented.
  • Effective communication and interpersonal skills.
  • Must maintain a high degree of unquestionable integrity in protecting confidential patient information.
Physical Requirements:
  • Individuals may need to sit or stand for long periods, occasional bending, stooping, lifting including patients, reaching forward, and overhead.
  • May require walking primarily on a level surface for periods throughout the day.
  • Proper lifting techniques are required.
  • Frequent computer work is required.
  • Must be able to travel freely in the local area; between clinics, hospitals, health departments, etc. Personal vehicle required.
As a Medicaid/Medicare facility, Carolina Family Health Centers must comply with the Federal COVID-19 vaccination mandates. Therefore, as of January 27, 2022, new and existing employees to have received, at a minimum, the first dose of the primary series (Moderna or Pfizer) or a single dose COVID-19 vaccine (Janssen), have requested a lawful exemption, or been granted a lawful exemption.
