I. POSITION PURPOSE:

Case management is a care delivery model that is focused on managing the components of care for members within or across the continuum of care, with the goals of achieving quality care outcomes and financial appropriateness. A case manager oversees the processes of care delivered to members, works collaboratively and provides leadership to the health
care team, and is committed to the organization’s goals for professional case management services.

II. ESSENTIAL DUTIES:

Coordinates and monitors services, including comprehensive tracking of member activities in relation to care plan.
Oversees the member’s plans of care. Provides patient teaching to members and families/significant others as defined by need.
Participates on quality of care reviews within the organization and with outside agencies. Coordinates the member’s ongoing care in conjunction with outside agencies as needed. Oversees the member's plans of care.

III. MINIMUM REQUIRMENTS:

Registered Nurse in Georgia or compact state.

3-5 years of CM experience.

IV. PHYSICAL REQUIREMENTS:
No or very limited physical effort required.
Work environment involves minimal exposure to physical risks, such as operating dangerous equipment or working with chemicals. Work is normally performed in a typical interior/office work environment.

Position is Full-time. $80,000 year - In Office

Job Types: Full-time, Part-time

Pay: $83,000.00 - $93,000.00 per year
