BayCare Health System is currently in search of our newest Case Management Assistant who is passionate about providing outstanding customer service to our community at Morton Plant Hospital. Morton Plant was the first hospital built in northern Pinellas County. The Case Management Assistant - Hospital, under the direction of Care Coordination department:
- Assist with completing tasks necessary to facilitate patient and family access to agencies, transportation services within and beyond acute care;
- Assist with distributing regulatory documents to identified patients
- Educate patients about the appeal process related to their discharge
- Perform multiple activities that support a seamless transition of patients from the inpatient hospital setting
- Knowledge of hospital processes and community service.
- Frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives.
