Case Manager
Provides ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. Responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, quality outcomes related to transition of care service delivery and compliance with external review agencies’ requirements .
Minimum Education, Experience, Training, and Licensures Required:
- Bachelor of Science in Nursing
- Current KY nursing licensure
- Five years’ experience in acute care nursing, case management, utilization management or related filed
- BLS
