Responsibilities

  • Participate in rotating on-call schedule.
  • Schedule at least weekly face-to-face contact with each youth and family served, preferably in their natural living environment.
  • Involve significant others in the youth and family’s treatment to augment or create additional supports for the family.
  • Convene/attend staffing as necessary for assessing and coordinating services for youth and family.
  • Functions as an advocate and support person to those individuals assigned to the CAT Team and link them to appropriate services and monitor the progress or lack of progress by the individual and his or her family.
  • Advocate in the family’s best interest.
  • Assess needs and develop a plan to meet those needs.
  • Monitor and evaluate service provision and individuals’ progress.
  • Transition services for youth nearing 18 years of age to an adult system of care.
  • Assist the youth and family to develop connections to natural supports within their own network of associates, including friends and neighbors, community organizations, including service and religious organizations and through participation in clubs, organizations, and other civic activities.
  • Arrange for procurement of incidental expenses of any additional temporary supports that may be necessary to move the family towards greater independence, stability and achieve resiliency goals as documented in their Plan of Care.
  • Assist in coordination of any adjunctive services that are out of the scope of practice and therefore not provided by the CAT Team Staff.
  • Assist with transporting youth or family to medical appointments, court hearings, or other related activities outlined in the Plan of Care.
  • Communicate and liaison with the individual, family and service providers Complete required agency, DCF, legal, statistical and fiscal documentation in an accurate and timely manner.
  • Provision of tutoring and remedial academic instruction to enhance educational performance.
  • Assist client/family with accessing community resources, medical and dental services, transportation, education coordination and tutoring, legal system advocacy and coordination, substance abuse/co-occuring services, family support network development, and employment/vocational services.
  • Responsible for providing respite care for up to 4 hours (no overnight).
  • Responsible for completing a thorough assessment of each client as directed by CAT contract.
  • Assist other team members with Crisis Intervention Services for the client and/or family.
  • Responsible for completing an individualized service plan for each client based upon information gathered from the assessment. The service plan must include the long-term desired outcomes for the client and must outline the comprehensive strategy for assisting the client in achieving these outcomes.
  • Responsible for timely completion and submission of time sheets, mileage sheets, progress notes activity sheets and any other required paperwork to direct supervisor each pay period.
  • Serve as an agency liaison with DCF, DJJ and other community agencies for coordinating referrals and organizing community presentations.
  • Conduct or participate in treatment teams, case reviews, case staffings, and peer record reviews as required.
  • Appear in court when subpoenaed as a witness.
  • Participate in 40 hours of in-service training annually. This includes completing any training identified as mandatory by the agency.
  • Participate in agency CQI/QA activities as assigned.

QUALIFICATIONS

Candidates will have graduated from an accredited university with a minimum of a Bachelor’s Degree in Psychology, Social Work, Marriage and Family Therapy, Mental Health Counseling or related Social Services or Behavioral Health field. Candidates must also possess at least one year of experience working with SPMI adults and SED children. Candidates must have some knowledge of the Medicaid manual and be willing to be certified by Medicaid within 90 days of hire. Three (3) years full time or equivalent experience working with children with serious emotional disturbances or three (3) years or equivalent full time experience working with adults experiencing serious mental illness. Training in the treatment of behavioral health disorders, human growth and development, evaluations, assessments, treatment planning, basic counseling and behavior management interventions, case management, clinical record documentation, psychopharmacology, abuse regulations, and recipient rights.

Other requirements:

  • Must have knowledge and must comply with state and federal statutes which affect the target population
  • Excellent verbal and written skills a must
  • Must have a valid FL Driver’s License and excellent driving record
  • Must successfully complete all criminal background screenings annually
