POSITION SUMMARY The case manager will be responsible for the daily coordination of treatment and milieu services for the resident including linkage and communication with the resident, treatment team members, referral agency personnel, and the resident’s family members.

SPECIFIC RESPONSIBILITIES include, but are not limited to: include, but are not limited to:include, but are not limited to:

Gather all necessary information to assure the intake assessment can be completed in a timely manner.

Participate in the intake assessment with the therapist. Either before or upon conclusion of the admission assessment take participants on a tour of the facility.

Work with the custodial agency/guardian to schedule an admission date for the client and assure that all necessary documents are completed, submitted and signed.

Be present at the time of admission to facilitate a smooth transition of the new client into the program.

Maintain frequent contact with caseworker, family members, and external treatment team members to communicate the client’s progress in the treatment program, gather any necessary information, and plan for the coordinated delivery of treatment services.

Attend team meetings on a weekly basis.

Meet with the therapists (team, art and AOD), cottage supervisors, school teachers, the nurse(s), school supervisor and recreation therapist on a regular basis to plan and coordinate clients’ treatment needs.

Attend appointments with the consulting psychiatrist.

Represent Oesterlen at court hearings, SARs, school meetings and any other off campus meetings for all clients on your caseload, as needed.

Attend and assist/chair quarterly treatment staffings for all clients on your caseload.

Insure approved incident reports and major unusual incidents (MUIs) are provided to custodial agencies and guardians as contractually and legally required.

Document all contacts with outside agency personnel through the use of progress notes, contact logs, or daily notes.

Coordinate and complete monthly progress reports for all clients on the caseload.

Work with the business office personnel to resolve billing difficulties between Oesterlen and the financially responsible outside agencies.

Schedule a discharge staffing to occur approximately 30 days before the resident’s anticipated discharge date.

Attend and facilitate the discharge staffing with the primary purpose being to firm up a discharge date, decide upon an aftercare placement, and assure necessary aftercare services are either already in place or that a responsible party is delegated to follow through with getting them in place.

Complete a discharge summary within 2 weeks of discharge for all residents on your caseload.

Assist in Performance Improvement activities as assigned.

Provide In-service training workshops with the training department as assigned.

Participate on agency committees as assigned.

22. Participate in Therapeutic Crisis Intervention training and successfully pass both the written and physical test.

23. Be physically capable of successfully performing a physical restraint without injuring the client, him/herself, or other staff members.

24. Have the stamina and endurance to keep up with a group of very active adolescents, both physically and emotionally for eight or more hours per day.

Any other duties as may be assigned by your supervisor, the residential director or the executive director.

Qualifications

A bachelor’s degree in social services is strongly desired. Experience working with adolescents with developmental disabilities, mental illness, autism spectrum disorders, and emotionally disturbed population is desired. Case managers must have very good written, verbal, computer, and organizational skills and be able to relate to and work well with people with diverse backgrounds.

Requirements for Electronic Health Records (“EHR”) and Productivity

1. Be proficient in the use of the agency’s Electronic Health Record system. This includes being both accurate and “timely* with the data entry required for EHR. Skills in using a desktop PC, laptop, or tablet are essential.

2. Enter client goals, treatment objectives, and notes in the EHR system.

3. Enter accurately and timely the information required for the agency to bill for services.

4. Be able to understand and effectively use reporting information provided by the EHR system.

*Timely defined as “data entry completed simultaneously in work with clients or within 24 business hours of work activity.

Physical Demands and Working Conditions

Work involves some standing, walking, running, lifting, pulling, and pushing heavy loads and sitting. Walking, standing, and driving are frequent requirements. A Supervisor must be mentally and physically capable of working in an environment with very aggressive adolescent clients who may attempt to physically attack and/or intimidate staff members and Supervisors. Supervisors must be physically capable of being physically active for eight or more hours per day. Pressure is moderate with planned periods of increased activity.

A physical abilities test is required to ensure the Case Manager is physically capable of performing a successful physical restraint within the guidelines specified by Cornell University’s TCI Training Course and without injuring the client him/her self, or other staff members, and to ensure the Supervisor has the stamina and endurance to be physically active for eight or more hours per day.

Some travel, generally same day and within the state will be required.

Job Type: Full-time

Pay: $27,187.00 - $41,424.00 per year
