The Case Manager is a very important and critical position. The Case Manager oversees all aspects of barriers that contribute to families facing homelessness. This position comes with a great deal of responsibility and the ideal candidate must be able to make sound decisions. This position reports directly to the Executive Director.

Responsibilities

· Provide case management to families facing homelessness.

· Identify and connect families to community based resources.

· Ensure families comply with Sojourner House and the Department of Housing and Community Development Uniform Shelter rules.

· Conduct family intakes, document case notes and develop individualized Rehousing Plans.

· Submit various documents and reports in a timely manner.

· Work closely with the Housing Advocate and Senior Program Coordinator.

· Network with other homeless providers and community-based organizations.

· Accompany families to meetings to access supports and/or services, when necessary.

· An understanding of the complexities and challenges of securing housing.

· Facilitate monthly house meetings and schedule family workshops and trainings.

· Fiscal management of escrow accounts.

· Attend internal and external trainings and meetings.

· Attention to detail and follow through.

· Must be able to analyze information and draw conclusions.

· Work one evening shift a week (11am-7pm or 12pm-8pm).

· Possess a Massachusetts valid driver’s license and reliable vehicle.

Required Experience

· Managerial and Case management experience.

· Proficient in MS Word, Google Docs, and the ability to learn Efforts to Outcome (ETO) software.

· Excellent organizational skills and the ability to multi-task.

· Strong written and verbal communication skills; must be able to professionally compose emails, notices and reports.

· Team player and a sense of humor.

· Ability to work well under pressure.

· Bilingual in Haitian Creole or Spanish (plus).

Job Type: Full-time

Pay: $50,000.00 - $55,000.00 per year
