Title: Case Manager

Reports To: Case Manager Supervisor

Location: Kingston Family Health Center

Education/Licenses Required: Bachelor’s degree in Social Work, Human Services or related field preferred.

Summary: Case managers screen children and families for insurance and other benefit program eligibility. Case managers will complete applications for eligible families. Case managers provide intensive tracking and follow-up with applicants to ensure that benefits are successfully accessed and all required supporting documentation is obtained.

A. Job Tasks

  • Works collaboratively with physicians, and other health care personnel in patient evaluations and treatment to further their understanding of significant social and environmental factors underlying patient’s health problems.
  • Provides direct case management services to assist patients in receiving necessary resources and services such as: Medicaid, Child Health Plus, Welfare, DAS, Social Security, Child Care, Home Care,

Housing/Shelter/Safe Homes, Substance Abuse Treatment, Mental Health Treatment, HIV Services, Legal, other Health Insurance etc.

  • Documents all patient services in patient’s electronic medical record, and closes encounters within 24 hours.
  • Prepares case management assessments, patient histories, service plan and reports.
  • Advocates when necessary for patients with outside systems.

B. Experience and/or Skills

  • Computer literacy with proficiency in MS Word, Excel, and PowerPoint.
  • Demonstrated organizational, interpersonal, oral and written communication skills and the ability to handle multiple assignments at any time.
  • Ability to generate, interpret and analyze data from multiple sources.
  • Bi-lingual, Spanish preferred.

Pay: From $19.00 per hour
