JOB SUMMARY

The Case Manager provides advocacy, assistance with housing searches, and supportive case management services to families residing in an emergency shelter community. Our objective is to not only assist families in securing adequate, affordable housing, but also to address the root causes which led to shelter placement.


JOB RESPONSIBILITIES

  • Provide advocacy, housing search assistance, and supportive case management services to homeless families
  • Provide life skills training to families such as budgeting, financial literacy, parenting, nutrition, etc
  • Assist parents with enrollment of children into Boston Public Schools system, including transportation
  • Make necessary referrals to appropriate human/social service agencies such as health care provision, legal services, mental health services, education/job training programs
  • Coordinate educational, recreational and extracurricular programming as needed
  • Serve as a liaison between families and designees of the Department of Transitional Assistance and/or the Department of Children and Families
  • Oversee enrollment and participation of children in Head Start after school and other youth programming and summer camps
  • Maintain detailed case files for all families served
  • Ensure compliance with scope of services as prescribed by the Department of Housing and Community Development
  • Conduct weekly case management meetings with all heads of households
  • Enter timely data entry into HMIS/ETO software for all families served
  • Draft and ensure performance of monthly rehousing plans
  • Ensure compliance with contractual obligations with both the Department of Housing and Community Development (DHCD) and Families in Transition (FIT) for all families served
  • Enforce rules, regulations, and policies of both DHCD and FIT
  • Maintain updated certifications required by both DHCD and the YMCA of Greater Boston
  • Attend all related trainings and meetings


JOB QUALIFICATIONS

  • Bachelor's Degree in either Human Services or Social Work preferred. Experience may be substituted for education
  • Knowledge of shelter system, housing, social services resources and public assistance benefits is required
  • History of providing direct care services to low income families
  • Ability to work harmoniously in a multi-cultural environment
  • Familiarity with HMIS/ETO software
