PATH is seeking candidates passionate about helping others make a positive change in their lives to join our Care Coordination team as the Case Manager in Orange County.

ABOUT OUR PROGRAM
The Care Coordination and Encampment Resolution program offers care coordination and outreach, through a multi-disciplinary team, to unsheltered individuals and high utilizers of Orange County resources and community services. The goal of the program is to link unsheltered individuals to shelter, supportive service and housing by providing continuity of care from street to stabilized placement.

WHAT IS A JOB TITLE
As part of the Care Coordination and Encampment Resolution team, the Case Manager will work to provide outreach, linkage, care coordination and retention services to individuals experiencing homelessness. Responsibilities include:

  • Conducting client intake and assessment for the program to identify clients' individualized needs for services.
  • Collaborating with clients to develop individualized treatment plans that address short- and long-term goals.
  • Providing strengths-based case management services and service coordination designed to assist clients in obtaining and maintaining stable housing and employment.
  • Providing field-based and mobile case management services to address all identified client needs including, but not limited to: Independent Living Skills; Money Management; Housing Stabilization; Community Integration; Employment Linkage; Benefits Establishment; Linkage to community providers for substance use, primary and mental healthcare.
  • Providing on-going case management support to assess client progress and ensure treatment plan outcomes are met or changed as necessary.
  • Conducting crisis and risk assessments in consultation with supervisor/ team or LCSW.

WHAT YOU BRING
We're looking for candidates that:

  • Effectively use technology and computers as a work tool, including proficiency with Microsoft Office software and experience with database systems such as CRM and/or EHR.
  • Maintain and execute confidential information according to HIPAA and all other applicable standards
  • Demonstrate strong problem-solving and conflict resolution skills
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs
  • Work just as well independently as they do on a team
  • Exercise mature judgement, and are highly motivated, self-starting and proactive
  • Are excellent at communicating, whether in writing or verbally
  • Have a strong sense of prioritization and can coordinate multiple demands in a high-pressure environment

PREFERRED QUALIFICATIONS
An excellent candidate for this role will:

  • HMIS Experience.

MINIMUM QUALIFICATIONS
To be considered for this position, we require all candidates:

  • Hold a Bachelor's Degree in Human Services, behavior, or two (2) years of direct service experience in homeless services or a related field.

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

DRIVING
Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 PATH will require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
