They are also one of the key factors that help individuals remain in the community versus transitioning to a care facility.

Key Case Manager Responsibilities:

• Coordinate services for senior citizens, determining needs and developing care plans
• Documentation of services into state reporting databases
• Leading with a positive attitude in all interactions with staff, clients, referring agencies, etc.

Qualifications for Case Managers:

• Bachelor of Social Work, or Bachelor of Arts/Bachelor of Science in Psychology, Sociology
• 1 year of experience in working with individuals with developmental disabilities, case management, nursing or social services
• Driver’s License for routine local travel
• Must be Vaccinated/Booster
