Home Instead in Somerset New Jersey, is looking for a CLIENT CARE COORDINATOR to become a part of our team and join our mission of enhancing the lives of aging adults throughout our community. The CLIENT CARE COORDINATOR will be responsible for managing new service inquiries, signing up clients, care consultations with clients and family members, and quality assurance visits.

Responsibilities:

  • Handle inquiry calls from prospective clients, family members, or other referral sources
  • Conduct care consultations to sign up clients, discuss their needs and provide solutions
  • Maintain ongoing communication and consistent follow up with potential clients and families regarding service and start dates. Work with clients to handle questions related to service, billing, rates, etc
  • Work with new and existing clients to increase service hours
  • Work closely with the Registered Nurse, Staffing Coordinator, and Recruitment Coordinator
  • Conduct Client and Caregiver introductions
  • Maintain client files and electronic client information
  • Develop and maintain positive relationships with clients/family members through quality assurance visits and phone calls
  • Support office team with administrative duties (phones, files, forms, etc)
  • Work with clients and insurance companies to resolve issues
  • Shared responsibility for on-call (with other admin staff) approx. 1/3 time. Additional compensation
  • Networking with referral partners

Qualifications:

  • Prior home care, health care, or senior-related industry experience required
  • Familiar with social media
  • Must be proficient in Word and Excel
  • Excellent written and verbal communication skills
  • Ability to meet deadlines and prioritize tasks
  • Ability to think outside the box
  • Have a “can do attitude” with a knack for problem solving
  • Must have the ability to work evenings or weekends as required
  • Extremely organized and detail oriented
  • Reliable transportation and the ability to travel
  • Must have a valid driver license and auto insurance
