Essential Functions of the position are detailed below, and include any physical requirements below that.
  • Manages a caseload of approximately 25 clients who present complex needs or require help navigating multiple systems. Clients will represent multiple life stages, from youth to adults to seniors. Navigator will also welcome clients, engage with staff and serve as a key member of the Catholic Charities community in the north and northwest suburban region.
  • Receives referrals from local program teams, mission engagement colleagues, and parish and community-based organizations. Conducts intakes and assessments to determine their needs. Prioritizes clients' needs and creates individual or family service plans with short- and long-term goals to help clients achieve more self-sufficiency.
  • Coaches and helps clients to advocate for needed supports and services (such as rent assistance, utility assistance, counseling, etc.). Promotes client's empowerment and self-sufficiency by educating them about community resources and how to use them.
  • Monitors client's progress to ensure they are on track to reach stabilization and are receiving the services they need.
  • Connects clients with resources both within Catholic Charities and outside of Catholic Charities.
Works cooperatively with others to ensure clients receive needed services on a timely basis.
  • Knowledgeable about programs offered across Catholic Charities, and by other providers in the region; builds relationships with those organizations to facilitate referrals and support of clients where appropriate. Works with the Charities Hub, Regional Operations Directors, and other Client Navigators to ensure Catholic Charities has updated information about services and partners in the assigned region.
  • Maintains and/or ensures up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing, or accrediting bodies.
  • Bilingual (English and Spanish) strongly preferred.
  • Works on other projects as assigned by supervisor or agency leadership.

Kneel and move from sitting, bending, kneeling or standing multiple times a day.
Push and pull objects up to 30 pounds.
Climb up and down up to 3 flights of stairs at a time.

  • Must be a problem-solver and a self-starter who can effectively manage an active case load.
