We are seeking a full-time Clinical Care Coordinator to join our awesome team at Aurora Mental Health Center (AuMHC)! Our clinical care coordinator provides a variety of individual services, case management, coordination, and collaboration both internally and externally to the benefit of clients. This role works in concert with the whole of the Connect to Care (C2C) team and across the broader organization, requiring excellent communication skills and the ability to balance a number of priorities.
About the team.
The Connect to Care team is the primary access point for clients beginning services at Aurora Mental Health Center. This is a multidisciplinary team, composed of positions that support clients in breaking down the barriers that prevent them from receiving care. This position plays an important role within the team, supporting the management of complex cases and projects that support clients directly.
Responsibilities:
- Provide high quality customer care, accuracy, and documentation in coordinating referrals with external and internal referral sources.
- Serve as primary liaison and provide relevant linkage for current clients with external partner agencies that also provide them care.
- Acknowledge receipt of referral, provide appropriate response, and communication with referral sources to enhance opportunities for future referrals.
- Document all necessary details regarding referral following established methods. Support creation of referral tracking mechanism that is embedded in EHR.
- Work in partnership with Customer Service Representatives and other team members on needed paperwork and enrolling new clients.
- Work with relevant staff to assign referral to appropriate provider, and interpreter (if needed), set up and coordinate initial appointments.
- Monitor and report on referral and coordination trends, and develop new strategies with identified staff to enhance the referral and care coordination processes.
- Ensure client access to needed services and community resources by problem solving barriers, including providing billable services if needed
- Respond to initial and follow up inquiries from referral sources regarding questions about referring to the clinic, status of referrals, or other requests.
- Assist with compiling data needed for grant and contract invoicing by billing department, or for other reporting purposes.
- Assist with other team special projects, such as coordinating administration of the NOMs, client satisfaction survey etc, as needed
- Provide support and training internally and externally regarding the referral process and accessing care, as needed
- Other duties as assigned
Requirements.
- Bachelor's degree in psychology, social work, or other related human services degree
- Preferred: 1 yr. of comparable job experience in social service, mental health or medical fields
- Knowledge of behaviors of clients with mental illness and/or emotional/behavioral issues
- Ability to assess crisis situations and intervene appropriately
- Knowledge of community resources
- Bilingual preferred
Salary for this role. Wage is based on experience and company equity.
$18.89 – $28.33 per hour.
Bilingual Language Differential Pay. *rate depends on language requirements for the position and candidate's fluency.
- 5% of base salary up to a maximum of $3,000/year
