Clinical Care Manager/ADON
Conducts or coordinates procedures to ensure timely completion of the comprehensive assessment (RAI) as specified by HCFA regulations. Provides coordination and supervision to ensure effective and timely resident care plans as specified by CMS regulations. Maintains and electronically submits all MDS records mandated by CMS regulations. Develops, implements, and monitors Health and Wellness Services by providing information, referral services, and promoting good health choices. Provides continuity of care and services to residents and clients by planning, organizing, directing, and controlling clinical areas. Plan, direct, or coordinate medical and health services in hospitals, clinics, managed care organizations, public health agencies, or similar organizations.
