Upperline's interdisciplinary team sees patients in the clinic, home, hospital and facilities and provides wrap-around services including 24/7 clinical support, remote patient monitoring, behavioral health telemedicine and other resources. We help provide relief from the burdensome symptoms of chronic illness, help patients and caregivers navigate the healthcare system, and guide patients through difficult treatment choices. Upperline Health is based out of Nashville, TN and has clinics in Alabama, California, Florida, Georgia, Indiana, Kentucky and Tennessee.

About the Clinical Dietitian
Upperline Health is seeking a Registered Clinical Dietitian to provide nutritional services to our high-risk, complex care patient population which includes counseling, nutritional assessments, prescriptions, and evaluation of nutrition-related lab parameters, as part of a member of the interdisciplinary team to improve results and maximize outcomes.
Ideal candidates will have experience working with Medicare patients. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment.
This position is hybrid remote and in-office in Nashville.
Responsibilities of the Clinical Dietitian
- Develop and train interdisciplinary team on dietary and nutrition guidelines to support health maintenance in older adult population with chronic illnesses
- Obtains and evaluates nutrition histories of the patients regarding food habits. Assesses patient’s knowledge of diet and health maintenance and provides appropriate detailed education to the patient and their family regarding nutritional status, adequacy measures, fluid status, and all nutrition-related lab parameters pertaining to their disease process.
- Completes written nutritional assessments as well as comprehensive patient assessments for the patients' written care plan in accordance with established Federal and State licensure guidelines. Participates in care plan meetings.
- Monitors patient’s adherence and response to nutrition therapy, and adjusts therapy as needed to achieve and sustain an effective nutritional status. Documents patient’s response to therapy in monthly progress notes.
- Collaborates with a multi-disciplinary team on the management of bone mineral metabolism, nutritional status, and fluid status. Participates in monthly Quality Improvement team meetings.
- Complete other related duties as deemed necessary by Upperline Health
Qualifications of the Clinical Dietician
- Registered Dietitian in at least one of the states Upperline operates in (CA, FL, GA, AL, TN, KY, IN)
- Licensed Dietitian Member of the American Dietetic Association is preferred
- Experience in adult clinical area is preferred, or at least 2 years work experience in adult clinical dietetics.
- Experience working with patients with complex, unmanaged Diabetes, Hypertension, and other chronic illness.
- Candidates should be excited and energized by the prospect of working collaboratively as part of an innovative team
- Prior healthcare experience serving chronically ill patients living with limited resources, serious emotional disturbance, and/or mental illness preferred
- Telephonic case management or care coordination experience preferred
- Strong customer-service orientation
- Patient, compassionate and professional demeanor required
- Exceptional organizational skills and ability to multi-task
- Self-starter with an entrepreneurial spirit and demonstrated problem solving skills
- Strong writing, Word, Excel, and database management skills
- Extremely strong work ethic
- Experience working in a fast-growing, rapidly changing environment
Compensation
Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience
