Summary/Objective

The NY START program offers training, consultation, therapeutic services and technical assistance to enhance the ability of the community to support eligible individuals and focuses on establishing integrated services with providers. Providing supports that help individuals to remain in their home or community placement is NY START’s first priority. Under the supervision of the START Team Leader, the START Coordinator is responsible for providing crisis support, linkages, outreach, and consultation services for individuals with a developmental disability and behavioral health needs.

Essential Job Functions

  • Participates in didactic trainings and additional trainings as required
  • Provides systemic consultation.
  • Participates in national Center for START Services trainings and events.
  • Prepares agendas and document outcomes for individual mentoring sessions held with Center for START Services
  • Maintains linkages and relationships with community partners.
  • Assists with referrals for consultation and treatment as needed

. Ensures the coordination of support meetings and crisis plan development for individuals with served through START.

  • Participates in recurring meetings with START leadership, clinical team, and resource center program.
  • Provides outreach visits to the individuals’ home, day, and vocational settings as needed

. Uses the START Action Plan for case planning and strategizing.

  • Develops Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans for resource center admissions or inpatient hospital, admissions and any other applicable documentation of services provided.
  • Maintains an active caseload of individuals referred for START Services (variability occurs depending on activity level of cases and the project scope).
  • Provides on-call support on a rotational basis as part of a START team.
  • Participates in the START Network, which brings together projects from across North America through an online resource center, conference calls, select study groups and a meeting at an annual conference as recommended.

Duties and Responsibilities

  • Obtains and maintains certification in PPD, CPR, First Aid, SCIP (Level 2), as needed per Agency policy.
  • Performs additional responsibilities as requested and/or necessary, including supporting alternate locations when assigned.
  • Adheres to flexible schedule that reflects the needs of the program(s).
  • Maintains on-call responsibilities, as required.
  • Attends in-service training programs, clinical and academic conferences and workshops and other educational presentations to maintain current knowledge of relevant clinical issues and techniques.
  • Addresses emergency situations as they occur in the program according to established policies and procedures, and ensure communication of such situations in a timely fashion.
  • Completes and submits all required paperwork, both paper and/or electronic per compliance regulations.
  • Ensures that electronic individuals’ records and daily data input are maintained

. Ensures all required documentation is maintained in records in the prescribed manner.

  • Attends and participates in staff meetings and in-services in accordance with Agency policy.

Education, Work Experience, and Competencies

  • Master’s Degree in social work, psychology, counseling, or other human service field required.
  • 2 years’ experience working with people who have IDD and mental health and/or behavioral health needs. Experience can be paid or unpaid working in the field.
  • Ability to work with complex systems and an interest in the population is essential
  • Independent, organized, and effective oral and written communication are necessary.
  • Experience working with families and individuals.
