Job Summary
Under the direction of the Quality Improvement Leader for Hoag Clinic, the Quality Improvement Supervisor is responsible for leading and facilitating quality improvement initiatives as content expert, quality improvement facilitator, educator, and consultant. The QIS is a key leadership member of the clinical team who oversees the quality of care in order to achieve the best outcomes and patient experience.

The Quality Improvement Supervisor displays and practices a work-style that reflects the mission, vision and values of the Hoag Medical Foundation as well as Hoag Hospital. Participates in the continuous Quality Improvement process, maintains a working knowledge of departmental standard operating procedures, including the use of specialized instrumentation, Quality Improvement requirements, and preventive maintenance.

Essential Functions
  • Oversees the day-to-day and operational goals of the quality improvement and senior wellness initiatives at Hoag Clinic.
  • Will provide supervision, training, and leadership to QI Assistants & Senior Wellness staff members to achieve team and Hoag Clinic goals.
  • Is able to effectively communicate with staff when there are performance issues, and is able to professionally address those issues in order to resolve them.
  • Continually review and evaluate the progress and performance of staff and design developmental programs tailored to meet their career goals. In conjunction with the Department leader, establishes yearly department goals.
  • Initiates new programs, enhances existing programs and continuously assesses need for resources, tools and additions.
  • Provides excellent and comprehensive communication and education to internal and external staff, providers, and community members.
  • Assists with Provider, staff and member training. May coordinate and oversee annual chart audits collection, HP chart requests, claim or payment resolution, etc.
  • Initiates member and provider communication relating to missing tests, measures and “due” reports.
  • Produces accurate staff and “measure” productivity reports in a timely manner.
  • Identifies areas that need improvement and initiates creative solutions (cost effective) to correct problems.
  • Ensures proper, timely, and accurate performance status to sites, leaders, and providers.
  • Produces accurate metrics reports as well as staff productivity reports in a timely manner.
  • Identifies areas that need improvement and initiates solutions to correct issues to meet department goals.
  • Responsible for overseeing the clinical quality improvement initiatives for Hoag Clinic and working with physicians and staff of primary and specialty care offices to ensure success in the programs.
  • Responsible for overseeing the necessary operational components of any ACO and PPO quality programs.
  • Is as the primary point of contact for physician offices regarding the Medicare Quality Payment Program (QPP), including the Merit-Based Incentive Payment System (MIPS), and the Integrated Healthcare Association (IHA) Value Based Pay for Performance program.
  • Builds and maintain expertise about the Medicare Quality Payment Program including future impacts to the organization.
  • Uses outcomes reports, trends, and benchmarks to strategize for quality and senior wellness initiatives.
  • Develops and implement educational tools for onboarding and ongoing training of providers and staff for all/any new quality initiatives.
  • Present information in large and small group settings related to quality and senior wellness metrics.
  • Oversees the monthly and annual health plan, IHA, CMS & ACO audits related to quality improvement and value-based programs.
  • Fully comprehend clinical quality measures and supports in ongoing communications to the staff and physicians
  • Works with the multiple teams, such as UM, pharmacy, Case Management, at Hoag Clinic to implement projects that support initiatives for shared savings/value-based programs.
  • Ability to travel to provider office sites and Hoag Urgent Care locations as needed.
  • All other duties as assigned.

Education, Training and Experience

Required:
  • 3 to 5 years of experience in the health care environment required
  • Associate of Arts (AA) degree in a related field
Preferred:
  • Bachelor’s degree in a healthcare-related field and a minimum of 3 years of experience in the health care environment, or Master’s degree in healthcare related field
  • Experience with Quality Improvement programs

Skills or Other Qualifications

Required:
  • Strong experience with reporting for the Incentive-based Programs
  • Experience with ICD-10, CMS (MIPS, MU, PQRS), and Accreditation (ex: AAAHC, etc.)
  • Demonstrated ability to achieve positive outcomes on quality improvement projects
  • Experience in teaching health care providers, clinical and non-clinical staff
  • Experience in teaching health care providers, clinical and non-clinical staff
  • Proficiency in the use of computer applications, including Microsoft Word, Excel, PowerPoint, and EHR systems
  • Excellent communication and presentation skills, including public speaking
  • Excellent project management and organization skills
