Position Summary: The Certified Nursing Assistant (C.N.A) will care for and assist in the care of assigned patients in the health care system. The C.N.A supports medical care as directed by the medical staff, registered nurses, and licensed practical nurses and pursuant to objective goals and policies of the facility.

Nature and Scope
: The C.N.A. will be given assignments and directions from LPN’s, RN’s and Physicians. The LPN is accountable for implementation of the nurse aide process for an assigned number of patients. The needs of the patient will be assessed by the LPN/ RN in collaboration with the physicians, staff, patient and patient’s family and by chart review to help determine any special activities that the nurse aide will be instructed to perform besides the standard nurse aide duties. The plan of care/ treatment plan will begin at time of admission and the C.N.A. will be aware of the plan of care and assist peers and co-workers in implementation. The treatment plan/plan of care will be continuously reviewed and revised as needed until point of discharge including a discharge plan of care. The C.N.A. is responsible for knowing the condition of their patients while on duty and communicating changes appropriately to the LPN’s and/or RN’s and /or physician and documenting appropriately. The C.N.A. is accountable to perform assigned procedures, which are designated by facility policy. The following are the basic criteria that the C.N.A. will be held accountable for:

The C.N.A will demonstrate the ability to carry out medical orders as directed and in accordance to policy and procedures.


The C.N.A will maintain professional and open communications with co-workers and physicians. They will demonstrate effective communication with patient and family members. They are accountable for cooperating with other personnel to achieve departmental objectives and maintain employee, interdepartmental and public relations. Maintains patient rights and confidentiality and complies with departmental and facility policy and procedures. Demonstrates effective verbal communications skills including phone manners.


The C.N.A is accountable for assisting in maintaining a safe and clean environment in all patient care areas. They will assure operational condition of all emergency and specialized equipment that might be used on their shift by checking the equipment at the beginning of their shift. Institutes cost effective measures including procuring charges and maintain records of nursing and medical treatments rendered, as applicable.


The C.N.A is responsible to maintain and encourage a positive and professional working relationship with employees, patients and families- responding to their needs with promptness and empathy. They should follow the chain of command, giving or expressing criticism of hospital personnel, procedures or policies in private and in a constructive manner promoting good will to and for the hospital and fellow employees.


The C.N.A may participate on committees and in services at departmental and hospital levels as assigned or volunteer.


The C.N.A is accountable for assuming other duties and other areas of responsibility as requested by the House Supervisor and / or Department Director.


Principle Accountabilities:
Displays awareness of legal implications in nursing practice by maintaining accurate nursing documentation, promptly reporting all situations with potential litigation to the department director, and maintaining a clean, safe and orderly patient care environment.


Maintains professional growth and development through professional affiliations, seminars, workshops, conferences, continuing education etc. Acts as a resource model for peers, students and other staff.


Complies with established nursing standards, maintains departmental and facility policy and procedures and demonstrates sound judgment in problem solving.


Position Requirement:
Current, unencumbered certification to practice as a C.N.A in the state of Oklahoma. Maintains current CPR and other certifications as required for position.


C.N.A. Shift Duties:
Receive shift report in a timely manner; patient care including assisting with showers, meals and other ADL’s as needed; interact with co-workers on unit and other departments in a professional manner; accurate, complete and timely documentation; (Bridges) assist with groups and making sure unit schedule is followed; answering phone- making sure that the caller’s needs are met or a number is obtained and message given to proper staff member; completion of unit logs as needed; maintain unit flow- make sure call lights are answered promptly and delegate tasks as necessary to appropriate staff; intervene as needed to help calm agitated patient or provide safety for patients and staff; discharge of patient-make sure all belongings are returned to the patient; assist in maintaining a clean and safe environment; provide snacks at allotted snack times and as requested, as appropriate; keep track and record intake and output; obtain vital signs as ordered and report abnormal readings; (Bridges) complete patient safety rounds in timely manner as ordered and complete unit safety round sheet; take out trash and keep linens stocked; report any concerns to RN/LPN; give complete shift report to oncoming staff in a timely and organized manner.


PRN Staff:
PRN staffs are required to view the monthly safety video and work a minimum of 2 shifts a month with one shift being on a weekend (ex. Saturday or Sunday). Failure to do so will result in termination.


Attendance
: Staff are required to show up to work in the proper attire, as scheduled. Staff must make sure that they all needed items to perform their job duties. No Call No Shows x 2 are automatic grounds for termination. You must call into the House Supervisor 2 hours prior to the start of the shift. If you are absent for 3 consecutive days due to an illness you must produce a physician release allowing your return to work. Excessive absenteeism may result in termination. All staff are required to watch the monthly safety video (unless they are on vacation or other extenuating circumstances) - failure to watch the safety video for three consecutive months will result in termination.


The C.N.A. must be able to lift 50 pounds, push 100 pounds, and endure prolonged extensive standing and walking. Assist with moving, lifting and repositioning of patients, supplies and equipment. Considerable reaching, stooping, bending, kneeling and crouching. Must be able to distinguish sounds, colors and odors.
