Co-occurring Counselor (Addictions)
Job description
- Maintain a caseload of up to 35 clients
- Conduct client screening, evaluations and intake procedures under supervision
- Complete Psychosocial History, Medical History, and Psychosocial Evaluation under supervision
- Complete treatment plans
- Provide direct individual, group, and family therapy to clients
ESSENTIAL POSITION RESPONSIBILITIES:
- Ensure that all elements of the treatment plan (including the aftercare plan) are realized within the applicable time
- Ensure that client records are kept in accordance with DDAP and agency policy
Observe clients in the program for changes in physical, emotional, and cognitive behavior and report such changes to the Treatment Team
- Assist in determining appropriate strategies, interventions, and treatment approach
- Document client progress and makes recommendations regarding discharge and aftercare planning
- Complete discharge summaries
- Interface with and maintains contact with significant others and referral sources, as required or needed
- Maintain appropriate rapport with a variety of additional service providers
- Conduct client follow-up and outreach
Minimum Qualifications:
Qualifications must adhere to DDAP Chapter 704.
- Bachelor’s Degree in psychology, social work or a related field preferred
- Bilingual ability preferred
FBI Clearance within one (1) Year
PA Criminal Background Check within one (1) year
PA Criminal Clearance within one (1) year
Health Assessment required
TB Test required
PA (ACT 31) Mandated Child Abuse Certificate required
Adult and Pediatric CPR Certification Required
Covid Vaccination record required
