Certified Nursing Assistant - Shahbaz

In the Green House Model of Long Term Care, the Certified Nursing Assistant is also known as a Shahbaz. The Shahbaz will care for the elders living in a Green House Home. Their primary role is to protect, sustain, and nurture the Elders by providing assistance with activities of daily living and meeting other needs as required. The core goal is to provide assistance in a manner that promotes a high quality of life and a positive Elderhood. The responsibilities of the Shahbaz includes care of the Elders and the Elders’ environment, including cooking, laundry, and housekeeping.

Job Description / Duties:

Recognize and respond to the needs of Elders

Promote autonomy of Elders in decision-making

Maintain confidentiality and privacy of Elder care, procedures, and documentation

Communicate with co-workers at all levels to meet the needs of Elders

Observe, monitor, and report to the nursing team any change in Elder's condition

Document observed data per organization policy

Assist Elders with personal care functions and activities of daily living based on Elder's preference

Measure and record vital signs such as temperature, blood pressure, pulse, respirations, weight, and height

Assist moving, positioning and transporting Elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.

Assist with the application of slings, elastic bandages, binders, etc.

Provide range of motion exercises as outlined and assigned by the care plan and therapy team

Prepare and serve meals, between meal, bedtime and other nutritional snacks per posted menu and the stated preferences of the Elders

Observe infection control and prevention standard precautions per organizational policy

Keep all areas of the home clean, performing routine housekeeping duties

Launder clothing, linen, and other articles

Follow established safety precautions and observe, monitor, and intervene in or report to the Nurse any unsafe conditions

Assure Elder calls for assistance are answered promptly

Report accidents and incidents immediately

Report any abuse or suspected abuse to the Administrator immediately

Attend and participate in orientation programs, on-going training, and educational classes

Perform other related duties as required by the Elder
