The Part Time Community Based Adult Services (CBAS) Registered Nurse (RN) conducts eligibility assessments using the CBAS Eligibility Determination Tool (CEDT) for referred LA Care Health Plan members. The CBAS center attendance is to promote living independently for those that pose a potential risk for placement in a Skilled Nursing Facility. The RN will conduct assessment to gather information by reading medical records and speaking with referred clients, client’s family member(s), and/or staff members working in the CBAS center. Topics of assessment include: diagnoses, medications, assistive/sensory devices, systems review, ADLs/IADLs (Activities of Daily Living and Instrumental Activities of Daily Living or basic self-care tasks). S/he is responsible for performing face-to-face assessments with older adults and/or adults with disabilities in the community. S/he will gather information to determine whether the individual meets the medical guidelines to attend the CBAS Center for day services. The RN will assess the client as a whole individual and be responsible for following up on any crisis related issues or medical concerns as needed.
Qualifications:
- Current RN License; must be active and unrestricted for the State of California
- Must have a reliable car, proof of auto insurance and valid unrestricted CA Driver’s License
- BSN preferred
- Experience working with the older adult population
- Experience working with adults with mental illnesses
- Experience working with adults with disabilities
- Experience with the California Dept. of Health Care Services CBAS Eligibility Determination Tool (CEDT), desired
- Proficient with Microsoft Office suite (Outlook, Word, Excel)
- Ability to function independently and with flexibility
- Community based service delivery experience preferred
- Must be culturally sensitive and able to work with a multicultural client/customer population; able to work in a non-judgmental manner and foster positive growth/outcomes; able to work as part of a team and maintain professional boundaries
- Ability to speak Armenian, Chinese (Mandarin), Farsi, Khmer (Cambodian), Korean, Russian, Spanish, Tagalog and Vietnamese desired but not required
- Must be able to pass criminal background check (Live Scan Fingerprinting) prior to beginning of employment
Responsibilities/essential functions:
- Must be willing to drive throughout West San Gabriel Valley
- Evaluate clients referred from LA Care Health Plan as a total person for functional and health limitations that impede independent living
- Responsible for conducting CDET Face-to-Face Assessments for CBAS eligibility determination
- Collaborate with CBAS staff and other health professionals to gather information
- Comprehensive and timely documentation of CDET form
- Ongoing interaction and collaboration with the CBAS Coordinator and CBAS Supervisor
- Other duties as assigned
