The Community Case Coordination Social Worker II position is responsible for conducting in-depth assessments covering psychological, social, physical, and environmental concerns for adults with disabilities and older adults discharged from a hospital, skilled nursing facility or other community-based care facility. S/he will evaluate clients, identify the potential limitations that create barriers to independent living, and activate resources to allow clients to remain safely in their home setting.
Qualifications:

  • Master’s Degree in Social Work or related field
  • Bilingual Spanish, Armenian, or Farsi is preferred
  • Experience and interest in providing care to adults with disabilities and older adults
  • Case management and community-based service delivery experience required
  • Knowledge of community resources and the aging process
  • Knowledge of safety issues when working with at-risk clients
  • Experience conducting in-home visits strongly preferred
  • Ability to function independently and with flexibility
  • Experience with Electronic Health Records systems desired
  • Must be computer literate in MS Office applications (Word, Excel, and Outlook)
  • Demonstrated ability to work in an interdisciplinary community setting maintaining appropriate professional boundaries
  • Culturally sensitive and non-judgmental in working with a multicultural client population
  • Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

Responsibilities/Essential Functions:

  • Conducts in-depth intakes and assessments covering psychological, social, physical, and environmental concerns
  • Collaborates with the client, their family, and the Community Care Coordination RN in the development of a comprehensive person-centered care plan including all social and medical services needed by the client
  • Identifies and develops family and other informal support systems for clients and refers clients to community resources
  • Coordinates the delivery of all services in the care plan and monitors quality
  • Follows up on client situations and offers crisis intervention as needed
  • Refers clients to specialist consultants to provide additional detailed evaluation and recommendations for improvement of the client’s functional level
  • Provides outreach to hospitals and skilled nursing facilities to educate potential participants about the benefits of the Community Care Coordination Program
  • Maintains comprehensive and timely documentation of all client contacts in the Electronic Health Record System
  • Supervised hours towards licensure available but not guaranteed
  • Other related duties as assigned
