Overall Summary
Responsibilities of the Bay Area Community Health (BACH) Community Health Worker (CHW) will include outreach and engagement to the Asian Pacific Community of Santa Clara and Alameda Counties, development of the COVID care plan, linkage of the patient to resources (food, housing, transportation, employment, benefits) as well as linking the patient BACH Services. Ideally, the CHW will be a trusted member of the community in which they will work. This relationship will enable the CHW to serve as a liaison for the patient in navigating health and social services. This position will have a high school diploma/AA or BA preferred, at least three years of progressively responsible work experience in a community-based health care service capacity, and will preferably be bilingual.
The CHOW will work within the Outreach Department Under the Supervision of appointed Program Coordinator.

- Advocate for patients and help provide COVID Awareness.
- Be a trusted member of the community.
- Be highly dependable and independent - much of the work will take place in various settings -medical office, community, hospitals, schools, churches and more.
- Communicate well with patients, document clearly in the medical record and interface with medical providers to advocate for the patient.
- Conduct outreach and enrollment assessments.
- Document patient interactions
- Connect patients to ALL COVID resources to meet these needs.
- Serve as a liaison between the community and BACH.
- Interface well with clinic staff and the community.
- Meet departmental review and documentation standards for work assignment.
- Adhere to the policy and procedures of assigned clinic(s).
- Build and maintain appropriate relationships on behalf of BACH
- Support a positive work environment and foster teamwork.
- Support a positive image of BACH inside and outside the organization.
- Participate in weekly meetings
Qualifications
Certificates and/or Licenses
- High school diploma/AA or BA preferred.
- At least three (3) years of progressively responsible work experience in a community-based health care service capacity.
- Bilingual preferred
