The Location:

Kalele Care Services was founded in 2020 by Ohana Pacific Health to address the complex healthcare needs of the community with an emphasis on assisting Kupuna to live longer and healthier independent lives.


The Position:

We're looking for a passionate Community Health Worker to assist and support clients with access to health and community services and facilitate annual wellness visits. The Community Health Navigator implements the patient's care plan to promote the health and well-being and adherence to care plan and clinical recommendations. Additional duties include:

  • If within scope of practice and training, the Navigator may perform ADL care as assigned.
  • Conducts annual wellness assessments by completing with the patient a comprehensive checklist, health questionnaire, records vitals.
  • Communicates changes in patient's status appropriately with the care manager and care management team.


Requirements:

  • High school diploma or equivalent.
  • Associates Degree in healthcare, business, communications, marketing, community health worker certificate, or nurse aide training preferred.
  • Experience in the healthcare or home health field; work with the geriatric population preferred.
  • Current and valid Driver's license.
