POSITION SUMMARY:
The MPPTI Community Health Worker (MPPTI-CHW) will serve as a key member of an integrated health team that works closely with our Teen Parent. The MPPTI-CHW will utilize their experiences and knowledge of the language and culture of the population he or she serves to facilitate bridging between teen moms and their families and the resources in the community. MPPTI-CHW can also help provide health education and information while working with Teen parents to effectively identify barriers to care and facilitate connections in the community to meet patients’ needs. The MPPTI-CHW should be comfortable with and have the ability to work effectively in a busy environment and demonstrate flexibility to meet the changing needs of the patients and the Health Center.
The MPPTI-CHW will participate in outreaching potential program participants, perform intake and enrollment, and provide case management. The MPPTI-CHW will meet with participants weekly and link them with programs and service at the health center and in the community. The MPPTI-CHW will implement MDPH’s tools (including the Life Plan guide) and strength base counseling to help participants develop personalized goals.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Engage in outreach efforts as determined by the Health Center and the Integrated Health Team
- Building a relationship with participants to identify and address needs related to social determinants of health such as housing and foods security.
- Conducting health, education/economic, and child development screening to determine participant needs.
- Provide supported referrals to participants to access healthcare, education, employment, and child development service as appropriate.
- Facilitates coordination of services and the family’s access to multiple resources within BNHC and the community, including:
- Comprehensive assessment of family social service needs
- Reducing barriers to health center behavioral health treatment
- Assistance identifying and accessing external community providers & social services
- Collaborate with MPPTI Behavioral Health Clinicians to address BH needs of program participants
- Communicate effectively with patients’ and their families, community partners, and with an inter-disciplinary team of professionals at the health center.
- Advocate for patients’ needs and support care coordination to meet those needs
- Complete all required documentation in a thorough and timely manner as required by Supervisor and per BNHC policy.
- Demonstrate HIPPA compliance and maintain patient confidentiality at all times.
- Attend all required meetings as determined by Supervisor
- Quality assurance
- Participate in weekly group supervision facilitated by a Supervisor.
- Participate in bi-weekly individual supervision with direct supervisor
- Participate in CHW training classes as determined by BNHC
- Complete all assigned my NetLearning Trainings
QUALIFICATIONS:
High School Diploma or Equivalent.
Bilingual or multilingual preferred
Medical interpreting certificate for CVC, Portuguese, Spanish, Haitian Creole or French preferred
MINIMAL KNOWLEDGE BASE:
Ability to read and write in English
Ability to use technology effectively
Understanding of the Brockton Community, available resources and needs of the population served
WORKING CONDITIONS:
BNHC is a busy community health center located in the downtown Business District.
Two late day per week may be required to accommodate patients attending evening appointments.
Travel is required
PHYSICAL REQUIREMENTS:
Physical demands requiring lifting of small pieces of equipment and/or boxes not to exceed 20 pounds.
Mostly sedentary and interoffice walking required.
Driving and walking in the community as required.
Visual acuity sufficient for frequent reading.
Hearing acuity sufficient for holding conversations with or without audio devices.

As a condition of employment, employees, to include new hires, are required to be fully vaccinated to include the most recent COVID 19 booster and annual flu vaccines, with the exception of those who have documented medical or religious exemptions, that must be approved by BNHC according to its protocols. Additional vaccines may be required based on the recommendation of the CDC. New hires who are not yet vaccinated for flu or COVID can arrange to be vaccinated on site.
