The Community Liaison position is VITAL in assuring that we meet and exceed the needs and expectations of both our clients and caregivers in our non-medical home care service organization. Professionally, you get excited engaging with people and winning referrals for the team. The Community Liaison we are looking for is one who is confident enough to ask for an opportunity to provide care.

Visiting Angels is actively pursuing an optimistic individual who can develop, foster, nurture and enhance relationships with a variety of community resources that engage and interact with our senior population in the greater Mesa AZ area. We are looking to develop relationships with those who could benefit from the services we provide as well as be a resource for our clients, family members and circle of support.

Community Liaison Responsibilities:

· Initial meeting with potential new clients to develop a care plan identifying all expectations of care by the client and their family

· Identify and facilitate opportunities for collaboration with potential referral sources to include education, presentations, events and any other format that will provide as well as be a resource for our clients, family members and circle of support

· Conduct “quality assurance visits” with current clients for quality assurance or care being provided. Provide case management type of services as needed.

· Engage with existing caregivers regarding their individual needs to deliver the consistent message that they are a highly valued member of the team

Community Liaison Requirements:

• Proven work experience in building positive, lasting relationships and growing private pay business
• Case Management experience a plus

• Health care experience is a plus

Pay: From $60,000.00 per year
