What is a Community Support Worker?

Community Living Support (CLS) staff work in our local communities to assist people with intellectual/development disabilities and/or mental illness to live as independently as possible within their own community. They encourage healthy lifestyles by offering exercise and healthy meal options. CLS staff also promote giving back to the local communities by encouraging individuals to volunteer, which can help with job skills when someone is looking to gain employment.

CLS staff focus on teaching life skills such as cooking, cleaning, the use of public transportation, and a variety of safety skills like “stranger danger”, pedestrian safety, kitchen safety, etc. Social activities are also a part of the CLS staff’s job, assisting individuals to participate in community events of their choosing.

A specialized branch of CLS, Behavioral Health Home CLS coordinate and provide access to community and social supports, as well as natural supports for individuals living in behavior health homes.

FAQ:

What is a behavior health home? A behavior health home (BHH) is a residence where individuals with chronic physical and/or behavioral health needs can get their services coordinated in one place.

Is there training for this position? Yes! NeMCMHA provides 100% paid staff training for our new-hires.

Do I need a license in order to apply? No! Though we do encourage people who have their CNA, DCW, or RTW training to apply, we provide all of the training necessary for this role.

QUALIFICATIONS

Education:

  • High School diploma or GED preferred.

Experience:

  • Knowledge of developmental disabilities and of people with developmental disabilities desirable.
  • Lived experience with mental illness, intellectual and developmental disabilities, and/or substance use disorders valued.
  • Ability to communicate effectively with consumers, families/providers, and community recreation providers in gaining access to inclusive leisure experiences.

Other:

  • Must be at least 18 years old.
  • Must possess a valid driver’s license and acceptable driving record (as determined by the agency).
  • Ability to work a varied schedule including evenings, weekends and holidays, and occasional overnight trips as needed.
  • Ability to maintain favorable public relations.

RESPONSIBILITIES:

BHH Community Support Workers will:

  • Assist the individual to increase or maintain personal self-sufficiency, facilitating goals of community inclusion and participation.
  • Assist individuals served gain access to participation in recreational, volunteer and other community based activities of their choosing.
  • Participate in the interdisciplinary team meetings and person-centered planning process.
  • Assist, prompt and support individuals served in activities identified in the Individual Plan of Service (IPOS).
  • Assist with preserving the health and safety of individuals served to support the most integrated and independent setting.
  • Community Living Support (CLS) activities will focus on improving independence and enhancing recovery.
  • Attend weekly BHH care team meetings to review and discuss active cases.
  • Identify community resources for initiative, facilitate health education groups, and provide education on health conditions including strategies to implement care goals.
  • Coordinate and provide access to chronic disease management include self-management support.
  • Demonstrate knowledge of universal precautions, standards of practice, and emergency protocol.
