Community Coordinator, SBHC Outreach & EngagementThe Fund for Public Health in New York City (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.
PROGRAM OVERVIEW
The Office of School Health (OSH) is a joint program of the Department of Education and the Department of Health and Mental Hygiene responsible for promoting the health of the 1 million school children enrolled in approximately 1,800 public and non-public schools in the New York City.
The OSH Adolescent Health Unit (AHU) oversees a variety of programs and services, including (1) The School-Based Health Center Program, overseeing 147 health centers operated by 20 sponsoring institutions, which offer comprehensive primary care services including mental health and age-appropriate sexual and reproductive health care as critical components of adolescent health care services; (2) the Connecting Adolescents to Comprehensive Healthcare (CATCH) program, which offers in-school pregnancy tests, emergency contraception, hormonal contraception and referrals for sexual and reproductive health care by OSH staff in school medical rooms; (3) the Mental Health Roadmap Initiative to provide training and technical assistance to school-based mental health professionals; and 4) Foster Care mental health initiative which utilizes innovative Eye Movement Desensitization and Reprocessing (EMDR) treatment modality to treat trauma among young people impacted by the COVID 19 pandemic and other losses and traumas.
POSITION OVERVIEW
Under the supervision of the Senior Manager, SBHC Outreach & Engagement, the Community Coordinator will provide onsite support to participating school-based health center sites, with the objective of increasing student engagement and the utilization of SBHC services. They will work collaboratively with the Program Manager to ensure that program activities are carried out according to the project timeline. Tasks will include educating adolescents about SBHC services, providing counseling on sexual and reproductive health, promoting SBHC services within schools, and coordinating outreach activities with SBHC and school staff.
RESPONSIBILITIES
- Coordinate and implement outreach and engagement activities in public high schools to engage adolescents in healthcare services at their school-based health centers.
- Provide in-person, text, phone, and email outreach about SBHC services to youth and families.
- Provide one-on-one and classroom-based education on sexual and reproductive health services, in collaboration with school and SBHC staff.
- Implement patient engagement survey at pilot sites to obtain adolescent feedback on SBHC services.
- Assist in the creation and coordination of outreach materials for the SBHCs.
- Maintain effective professional relationships with school staff, school-based health center staff, colleagues and supervisors to meet the needs of the program.
- Maintain knowledge of adolescent health and sexual and reproductive healthcare in order to provide up-to-date information and improved care to students.
- Complete all data entry and reporting for outreach and engagement activities.
- Participate in calls, meetings, and conferences with program partners, as appropriate.
- Travel to school sites throughout the 5 boroughs.
- Bachelor’s Degree from an accredited college or university
- Experience in health education and youth engagement
- Experience working in a school-based of adolescent clinic setting
- Excellent interpersonal, communication and presentation skills
- Excellent knowledge and experience in sexual and reproductive health topics
- Proficient in Microsoft Word, Excel, and Powerpoint
- Ability to work as part of a team, as well as independently
- Bilingual proficiency preferred
- Hourly range is $67,000 - $72,000

RESIDENCY REQUIREMENT
