Purpose:

The Coordinated Entry Intake Specialist serves as the initial representative and point of contact for clients seeking services within the Rapid Rehousing Coordinated Entry project and Permanent Supportive Housing. The CE Intake Specialist will conduct eligibility and needs assessments with individuals and families experiencing homelessness in addition to domestic violence and/or sexual assault. Coordinated Entry (CE) is an important process through which people experience or at-risk of homelessness can access the crisis response system in a streamlined way, have their strengths, needs, and barriers to housing assessed using a standardized assessment tool, and quickly connect people to appropriate housing or mainstream services.


Job Duties:

  • Assess needs (met and unmet), abilities, strengths, barriers and preferences of individuals seeking services and support through the in-take assessment interview. This maybe through in-person, telephone or other means of communication that are safest for the client. Review all available services with clients and link clients to internal or external services for scheduled appointments or selected partners for services. Provide supportive, trauma-informed crisis response to individuals impacted by domestic and/or sexual violence.
  • Respond in a timely fashion to inquiries and request from supervisor and partner agencies.
  • Complete and maintain accurate and current client records (including but not limited to data entry), as required by the agency, its funding sources, and/or other entities specific to the Rapid Rehousing and Permanent Supportive Housing grant guidelines and program compliance.
  • Generate required forms, reports and any other documents used in the take process.
  • Participate in all required organizational, program specific and position-related professional development, in-service trainings and direct supervision meetings. Stay abreast and increase knowledge of the domestic and sexual violence fields by reviewing current literature, best practices, and evidence-based programming.
  • Meet with internal and external collaborators and members of the Continuum of Care (CoC) on a regular basis to share service provisions and develop appropriate referral/in-take processes.
  • Perform other duties as assigned by Supervisor. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or is a logical assignment to the position.

Formal Education and Experience:

  • Graduate Degree in the field of Social Work is preferred working in a nonprofit organization with prior direct client services experience for high risk populations experiencing homeless, substance use, mental health and chronic health.
  • Experience working with survivors of domestic violence and/or sexual assault including dating violence, stalking, human trafficking, or other dangerous or life-threatening conditions that relate to violence against the individual or a family member, including a child.
  • Experience in community networking, case coordination for multi-disciplinary teams, providing a safe environment for women and demonstrated organizational skills required.
  • Knowledge of Housing First and harm Reduction philosophies and implementation.
  • Additional consideration will be given to candidates who are bilingual or possess related advanced certifications or licensure.
  • Completion of domestic and sexual assault violence training including safety planning, lethality and suicidal assessments.

Knowledge, Skills, and Abilities:

  • Proficiency in Microsoft Office (Word, Excel, Outlook) and the following databases: Homeless Management Information Systems (HMIS) and Efforts to Outcomes (ETO).
  • Candidate is required to complete a background check.
  • Ability to provide culturally competent service and work with diverse staff and clientele in an empathetic and non-judgmental manner.
  • Must demonstrate acceptable level of maturity, emotional stability and sound judgment to provide effective services to diverse populations.
  • Must possess excellent written and verbal communications skills.
  • Excellent interpersonal skills and ability to function as a productive, contribution member of a team, and work in a spirit of deep collaboration and partnership with colleagues as well as outside agencies.
  • Ability in integrate feedback and utilize supervision.
  • Ability to develop and maintain professional relationships with volunteers.
