Job Summary

Compile data needed to support housing needs of the community. Communicate with case managers to understand housing opportunities in the community. Refer homeless households to community services utilizing Coordinated Entry System (CES) Policy and Procedures.

Works under the supervision of Director of Social Service Programming.

Essential Job Functions-Responsibilities

1. Sustains strong working relationships with housing resources: CoC members and other providers of services to the homeless and those at risk of homelessness.

2. Assist in the ongoing development and implementation of the CES screening tool(s), policies, procedures, and processes.

3. Conducts in person and virtual training for all CoC access points and homeless service agencies related to HMIS, assessment tools, and other Coordinated Entry specific information.

4. Receives and makes referrals for the coordinated entry system as needed. Generates the Prioritization List (PL) of homeless households seeking housing opportunities through particular funding sources.

5. Update client records as needed to maintain accurate records and lists.

6. Maintain required system statistics and submit reports as requested.

7. Attends CoC meetings, CES, HMIS, etc. meetings as scheduled. Attends other community meetings as necessary and directed.

8. Facilitates semi-monthly meetings to case conference regarding clients and agency referrals.

9. Provide ongoing updates and feedback to their supervisor and the CoC related to the Coordinated Entry System.

1. Education: A.A. in human services field preferred.

2. Licensure: N/A

3. Experience: Data base management required. Experience in working with low-income families through a diagnostic and counseling approach, case management experience preferred.

4. Essential Technical/Motor Skills: Computer entry and typing.

5. Interpersonal Skills: Ability to effectively interact with clients, co-workers, and volunteers with diverse ethnic and cultural backgrounds and treat each individual with respect, dignity, and compassion.

6. Essential Physical Requirement: Ability to lift and carry 50 pounds; Ability to position self to access lower and upper shelves, cabinets, and files. Ability to ascend and descend stairs.

7. Essential Mental Abilities: Ability to assess client’s problems and potential and make judgement, good knowledge of resources available, organizational skills. Must be able to demonstrate, through case record keeping, appropriate use of social case work methods to achieve results.

8. Essential Sensory Requirements: Ability to communicate effectively, both verbally and through writing case stories of successes achieved. Ability to communicate requirements and program specific needs to other members of the community.

9. Exposure to Hazards: Some non-invasive client contact, exposure to typical office substances.

10. Other: Valid Missouri driver’s license, provide own reliable transportation and auto insurance. Ability to stick to a regular, reliable schedule each week.

Job Type: Part-time

Pay: $16.00 - $17.00 per hour
