The Crisis Case Manager position provides quality and timely case management services through a strength-based approach to clients experiencing a crisis, in support of the client stabilizing from a psychiatric emergency, adapting to life situations, connecting with natural and community resources, and preventing or reducing hospitalizations.

Essential Job Duties:

  • Provides direct crisis case management services to children and their support systems experiencing a psychiatric crisis, in an effort to stabilize individuals and integrate or maintain them in the community.
  • Provides first contact with clients experiencing a psychiatric emergency during regular business hours, assesses appropriate level of services for individual’s needs, and coordinates services with internal service providers.
  • Initiates documentation for emergency clients, such as composing drafts of progress notes, screenings, treatment plans, or referrals for completion by emergency service therapists.
  • Reviews cases as a part of the crisis access team when clients are released from an inpatient psychiatric hospital or have stabilized after crisis house placement.
  • Consults with emergency services therapist to develop an Alternative Safety Plan or consider client’s return to a secure placement. Assists client to identify and secure resources within the agency and community.
  • Serves as hospital liaison for clients being released from an inpatient facility by contacting the hospital for anticipated release date, requesting appointment times from the therapist and medication clinic at the Center, reporting appointment information to hospital staff, and recording the information in the client’s medical record.
  • Records and monitors contacts to ensure referrals, screenings, email notifications, and progress notes are completed in a timely manner. Summarizes data on a weekly basis for review by emergency services team.
  • Maintains accurate and complete records of all client services, as defined by state licensing agencies, funding agencies, and Mental Health Center standards and requirements. Submits daily appointment sheets, progress notes, treatment plans, timesheets, and all work-related paperwork in a timely manner and in acceptable format as determined by agency requirements.
  • Develops knowledge of community resources and how to access these services for the benefit of clients.
  • Establishes and maintains a positive working relationship and professional demeanor with clients, external community partners, CCBHC staff members, and visitors.
  • Attends professional workshops and seminars in order to maintain and improve professional skills and meet licensure/certification requirements.
