At PCS Mental Health, it's our mission to enable individual persons who have serious mental illness, co-occurring substance use disorder and/or a dual diagnoses of SMI and IDD live a life that is meaningful to her or him in the community on terms she/he defines.

Brief Description of Program:

PCS Mental Health, LLC will operate both a Supported Independent Living (SIL) program as well as a Community Residential Rehabilitation (CRRS) program for Carbon, Monroe and Pike Counties. These services shall function to help individuals learn to live successfully in the community in the least restrictive environment possible. In the SIL, staff will provide support to members living interdependently in apartments. The SIL program will consist of three (3) separate apartments which will accommodate up to six Members living in the community with staff support. The CRRS is a six-bedroom home staffed 24x7 to provide members the support, education and knowledge to live within the greater community. Each member will identify his/her own Blue-Sky, life-worth-living goal(s) and use these as inspiration in developing their unique Recovery/Treatment Plan.


Summary
of Job Duties:

This position will support the member and the team by understanding the goals on the Recovery Plan and implementing strategies necessary to attain those goals. This position will be front line, direct care support for the members and will work to ensure their safety. This position will also help support Members as they attain skills with ADLs and IADLs. This team member will participate in crisis management techniques and utilize de-escalation skills to help ensure a therapeutic milieu.


Specific Responsibilities:

  • Implement and document member interventions and progress relative to member’s recovery plan
  • Provide supervision for assigned member activities and trips
  • Provide training in and supervision of activities of daily living with members
  • Participate in groups as assigned
  • Provide individual support for assigned members
  • Provide specific training to members relevant to their assessed skills deficits
  • Conduct and document member face to face observation per P and P
  • Conduct and document assigned apartment checks per P and P
  • Prepare shift notes for assigned members
  • All documentation to be completed on day service is provided
  • Monitor and document self-administration of medications by members
  • Provide and document observed alcohol and drug screens per P and P

Qualifications:

  • High school Diploma or GED required, Bachelor degree preferred
  • Must have an understanding of Mental Health Recovery
  • Must have a valid Driver’s License
