Job description
- The DCS/School Liaison will serve as the intake worker for these service lines. The Liaison will track all referrals from DCS until their conclusion with services from the Samaritan Center. The Liaison will ensure that all school referrals are set-up in the EMR and evaluations schedule with all paperwork completed. The Liaison manages all the authorizations for CMHI/CMHW Wraparound and meets all the requirements set forth by the Department of Child Services.
- DCS: Understand the Service Standards for all services provided by the Samaritan Center StaffMee. Manage all referrals within 48 hours of receipt. Set up charts and schedule services. Serve as the communication hub between the DCS FCM & the Samaritan Center. Collect and manage data about referrals. Upload all DCS monthly reports.
- Schools: Receive referrals from the school-based staff. Communicate with family about services and expectations of paperwork. Schedule appointments in the therapist’s appointment book. Manage information in the client charts in order to ensure timely reports. Manage timelines and all requirements to ensure prompt payment for services. Assist in training staff about requirements for serving DCS clients
- Experience: Two years of related field experience preferred
- Education: Associate’s degree
