Job Summary:
CareLinx by Sharecare and OnLok PACE will be partnering in recruiting personnel which will coordinate the dietary services of Day Health Center and offer direct services to On Lok participants and allied health professional team members.
Essential Job Functions:
- Visits and observes participants in order to evaluate their nutritional needs. Reviews diet orders in charts and confer with physicians, nurses, and participants' family, when necessary. Completes dietary assessment and quarterly progress note for participants as required.
- Works with and participates actively in the interdisciplinary team regarding diet and nutritional problems and management, including participant care plans, problem solving and case review. In collaboration with Center/Program manager and other specialists promotes staff development in the area of nutrition.
- Provides nutritional education to participants, family and staff as needed; coordinates use of community resources. Provides information to the Nutrition Services Manager concerning preparation of meals for participants of the Day Health Center including menu planning, diet textures, and nutritional planning.
- Fulfills all requirements, such as reporting, keeping statistical records, making entries into charts and attending meetings as required.
- Other related duties as assigned.
Qualifications:
- Graduate of an approved Dietetic program.
- Registered by the Commission on Dietetic Registration.
- Minimum of two years as a Registered Dietitian in a clinical setting with experience working with the frail elderly or geriatric population required.
- Ability to work independently, as well as with the interdisciplinary team.
- Experience in leadership and training.
- Flexible, respectful, resourceful, self-motivated, and professional.
- Bilingual ability is preferred for specific assignments.
- California Driver's License and willing to drive and or travel for participant visitation (preferred).
Job Types: Full-time, Part-time
Pay: $41.00 per hour
Schedule:
- Monday to Friday
- Weekend availability
