To provide individualized support to members with disabilities and/or their families, in their own homes and community settings in compliance with all regulations and policies related to Home and Community Based Services (HCBS).

ESSENTIAL JOB FUNCTIONS:

● Assist members as necessary with a variety of daily living activities such as performing hygiene routines, toileting, cooking meals, lifting/transferring, learning a new skill, behavioral supports, supervision, transportation to activities, maintaining a safe environment, cleaning, attending community programs etc.

● Provide specific service supports as identified by the member’s unique PCSP (Person-Centered Service Plan) in one or more service area for each assigned member:

a. Attendant Care (ATC): A service where providers give support to members in their own homes in areas of daily living and hygiene. ATC providers must have Direct Care Worker (DCW) training. ATC providers also maintain service logs for each ATC activity during visits.

b. Habilitation (HAH): A service where providers give support by teaching and reinforcing new skills related to a member’s independence, development, and safety. HAH providers also maintain service logs for each HAH activity during visits.

c. Respite (RSP): A service where providers support a member’s parent(s)/guardian by providing supervision for the member either in or outside of their home. This service allows the member’s immediate caregiver an opportunity to care for other family members, attend to personal needs or relax.

D. Medication Administration: Depending on need, additional training may be given to allow providers to assist with certain medication administration.

E. Limited, Non-Nursing Medical Support: Depending on need, additional training may be given to assist members with non-nursing level medical needs such as changing diapers, assisting with g-tubes, utilizing durable medical equipment (DME) etc.

The quantity of services allowed per member is predetermined by the member’s PCSP. Services are quantified by hours per week, and certain types of services may be subject to daily limits. Providers may be assigned to more than one type of service per member, and more than one member per week depending on the provider’s desired careload. The total number of hours worked by an individual provider cannot exceed 40 hours per week, despite the number of service hours available per member or total assigned members.

Ultimately, providers are responsible for supporting the emotional, physical, and mental well-being of the member.

● Promote the rights of people receiving support and maintain an environment free of abuse, neglect, intimidation, or exploitation.

  • Adhere to all company and state policies

Qualifications

● Must have at least 3 months experience and/or education in the field of developmental disabilities or related. Acceptable experience includes but is not limited to caring for a family member with a disability, previous employment as a provider or a caregiver, a special education teaching degree, paraprofessional certification, volunteering with a disability-related community organization etc.

● Must be at least 18 years of age.

● Must be able to pass a background check.

Must complete the minimum necessary training classes (~10 hours, paid for by Absolute)

● Must be able to acquire a Arizona Department of Public Safety Level 1 Fingerprint Clearance card (paid for by the applicant, approximately $75)

  • Must have reliable transportation to/from the assigned member’s location of service
  • Must have reliable transportation to transport members within the community who require transportation assistance during assigned shifts, OR complete a non-driver agreement if you do not wish to transport members.
  • Must have a smartphone in order to clock in and out of shifts at the member’s home or in the community.
  • May be required to perform physical duties such as lifting, bending, kneeling, walking etc.
  • Shifts available in 85143, 85204, 85345, 85009, 85305, 85032, 85381, 85304, 85395, 85382, 85044, 85045, 85210, 85259, 85048, 85142 and 85044.
