Assist the interdisciplinary team working to prevent spread and reduce the impact of communicable diseases in the Lincoln County Community. Investigate disease cases and outbreaks as well as develop relationships with affected populations and community partners to improve disease detection and mitigation efforts.

Serving as a frontline disease intervention specialist who acts as a liaison, link, intermediary, and navigator between health and community resources to facilitate access and to improve the quality and cultural competence of service delivery
Essential Functions/Major Responsibilities
Disease Investigation, Intervention and Partner Services:
- Conduct full epidemiological investigations, including but not limited to conducting individual case interviews of reportable conditions, contact tracing, cluster identification, and outbreak investigation.
- Provide education, consultation, and guidance to cases and medical providers on screening, testing, and treatment, and initiates actions to mitigate the impact of communicable diseases.
- Receive and interpret lab results and medical records for use in case investigation.
- Contact potentially exposed persons to encourage testing, treatment, provide education, and obtain information on other potentially infected persons.
- Build and maintain working relationships with key community partners, such as schools, congregate care facilities, and healthcare providers.
- Work with community partners to implement communicable disease prevention and control measures in outbreak settings.
- Communicate by establishing rapport, active listening, using open-ended questions, and communicating at the patient’s level of understanding.
- Provide educational presentations and consultations to medical providers, substance abuse treatment counselors, community groups, and school staff and students on communicable diseases.

Specifics: CPR certification within 30 days of appointment. Must obtain Community Health Worker Certification within one year of appointment.

Desirable Qualifications: *
Disease Intervention Specialist 1 & 2
Fluent in Spanish
Bicultural competencies for the Latinx and Hispanic population preferred.
Disease Intervention Specialist 2
Two years’ experience at a level equivalent to Community Health Worker or DIS.

Knowledge of: *
- Extensive knowledge of the Lincoln County Area.
- Principles, policies, procedures, methods, and techniques specific to infectious disease control, prevention, and public health.
- Interviewing and motivational counseling techniques and principles.
- Medical and drug terminology.
- Communicable disease infections and their health effects, modes of transmission, prevention, treatment and effect on families and social institutions.
- Computer tools and software such as Microsoft Power Point, Word.

Job Conditions: This position primarily works some combination of hours Monday through Friday with the possibility of hours in the evening or on weekends.

The County's current contribution rate is 11%.
Regular and Trial Service employees working between 20 - 39 hours per week on a regular basis are eligible for pro-rated employee benefits.
