Early Intervention Service Coordinator
A typical day in the life of an Early Intervention Service Coordinator involves coordinating the eligibility process and subsequently individualized family service plans for a caseload of approximately 65 families. This is accomplished by meeting with or otherwise communicating with families to determine areas for developmental opportunity for their child and coordinating the funding, resources, and specialized services to be provided. Communicates frequently with Early Intervention service providers on the outcomes for each family and the steps being taken to accomplish them. Regularly updates case notes for families served with this information and participates in weekly meetings with Hamilton County Developmental Disabilities Services' Early Intervention team to present on these updates.
Job Duties (Summary):
  • Assists parents of infants and toddlers with disabilities and/or developmental delays in obtaining access to needed Early Intervention services and other services identified in the Individualized Family Service Plan (IFSP), including making referrals to providers for needed services and coordinating appointments;
  • Coordinates the provision of early intervention services and other services (such as educational, social, and medical services that are not provided for diagnostic or evaluative purposes) that the child needs or is being provided;
  • Coordinates, facilitates, and monitors the delivery of services required under the Individuals with Disabilities Education Act (IDEA) part C to ensure that the services are provided in a timely manner;
  • Coordinates and participates in evaluations and assessments as appropriate;
  • Facilitates and participates in the development, review, and evaluation of IFSPs;
  • Conducts referral and other activities to assist families in identifying available Early Intervention service providers;
  • Conducts follow-up activities to determine that appropriate IDEA part C services are being provided;
  • Informs families of their rights and procedural safeguards, as set forth in IDEA part C and
related services;
  • Coordinates the funding sources for Early Intervention services on an IFSP;
  • Facilitates the development of a transition plan to preschool, school, or, if appropriate, to other services;
  • Maintains records of the child, family and referrals in a confidential and up-to-date manner; and
  • Completes Early Intervention Data System (EIDS) data entry and other required early intervention forms necessary to document all service coordination activities.
  • Travels, as needed, to various locations for the purpose of service planning and monitoring; travels in a safe, efficient and effective manner in accordance with the County’s established procedures.
Requirements (Education, Experience, Licensure, Certification):
  • Bachelor's degree required in audiology, child and family studies, child development, child life, counseling, education, hearing and speech sciences, human development, human ecology, human social services, medicine, nursing, nutrition science, occupational therapy, ophthalmology, optometry, physician's assistant, physical therapy, psychology, rehabilitation, social work, or speech/language pathology.
  • Eligible to obtain the Ohio Department of Developmental Disabilities Early Intervention service coordinator credential
  • Valid driver's license; meets Board's requirements for driver insurability
