On a daily basis participates in supported employment services by performing the following duties. More duties listed on the job description.
- Delivers Discovery and/or Planning Services to Individuals Seeking Integrated Employment.
- Participates in activities related to career exploration and ultimately authors a plan for pursuing employment.
- Develops effective strategies for individualized job development/placement/support.
- Develops and expands community collaborations/partnerships.
- Provides prospective employers with appropriate support and information; responds appropriately and respectfully to employer’s concerns regarding hiring individuals with disabilities to include needed accommodations, when necessary.
- Works closely with the business developer to provide all individuals with access to Community Based Assessments (CBA) and individualized job openings.
- Assists in developing individualized skills necessary to seek, obtain and maintain employment.
- Complete required documentation and paperwork in accordance with Department timelines.
- Make appropriate support and service recommendations based on casework completed with individuals on caseload.
- Provides Individualized Job Support Services and Placement.
- Assists individuals seeking competitive, integrated employment.
- Teach job seeking skills, through individual instruction or counseling, including information on employment techniques, application assistance, resume preparation, communication skills, socialization skills, job maintenance skills, and assistance with job placement.
- May provide transportation to individuals on caseload with approval of supervisor.
- Advocate on behalf of individual for accommodations needed for successful employment. Performs routine follow-up with the employer and individuals to address any issues that may arise.
- Provide required documentation within Department established timelines.
- Comply with all HIPAA related considerations.
- As needed, will assist individual with performance on new job tasks, assist in covering the workplace culture to the individual, and outlining the employer’s expectations on-site.
- Ensure a warm hand-off of the individual to the job coach once placement is determined and continue to check in weekly with job coach to assess need for supports.
- Ensure that job coach is an acceptable match for both individual and employee prior to placement start date.
QUALIFICATIONS:
- BA/BS in Human Service Field and two year of related work experience or an AA/AAS in related field plus four years of related work experience.
- Experience working with individuals with disabilities in vocational rehabilitation and/or training settings is desirable.
- Must complete Funding Source required training hours within ninety (90) days of employment.

SHIFTS: (FT 40 hrs.) Mon-Fri: 8am-5pm
PAY RANGE: $21.98-$26.97/hr.
