GENERAL DESCRIPTION:
The Co-Options program is social/recreational program for children with intellectual and/or developmental disabilities and their parents/guardians. Parents/guardians attend Co-Options activities with their child. The direct support professional shall provide support and guidance to parents/guardians and children, ensuring consumers are cared for under the best practices and support options. The direct support professional will work with the Family Support Services Manager to plan activities/events monthly and will be responsible for all necessary paperwork for all activities/events. Additionally, the direct support professional shall assist individuals with Intellectual and Developmental Disabilities in participating in recreational and social activities within the community and will assure that their health and safety is maintained during community outings. The DSP will always treat the consumer/caregiver with dignity.

Co-Options programming occurs on Saturdays and requires in-person staff attendance. Saturday events take place from approximately 10AM – 2PM. Activities usually run for 2-3 hours and activity locations are in Morris and Essex county.

DUTIES AND RESPONSIBILITIES:

Maintains regular communication with Family Support Services Manager via agency email

• Direct Support Professional must work 3 in-person events per month, for 2-3 hours
• Be attentive, flexible, and supportive of consumers and their families
• Encourage and model social behavior and appropriate self-expression
• Collaborate with Family Support Services Manager to develop program activities that align with participant needs
• Facilitate program activities and coordinate supplies when applicable

• Responsible for maintaining the comfort, health, safety, and well-being of consumers during program events and intervening when necessary
• Provide supervision to individuals in the program

• Submit all program documentation in a timely manner, including attendance sheets, timesheets, Level of Service (LOS) notes, debit card reconciliation and receipts, etc.
• Maintains CPR and First Aid certification (every two years) and other job requirements as necessary
• Prepare yearly self-evaluation for supervisor and undergo an annual review process
• Attends quarterly DSP/program meetings and other agency events as required
• Must take action and inform Manager as soon as possible when:
• an injury occurs during program activities/events
• filing a neglect and abuse complaint
• recording an unusual incident

• Follows mandatory reporting procedures according to FRN and state protocols
• Respond immediately and appropriately to ensure consumer health and safety needs are met
• Must be able to engage in physical activities including walking long distances
• Maintain mental and physical alertness and an appropriate level of energy to perform essential job functions
• Other duties as assigned by supervisor and/or CEO

REQUIREMENTS:

Must be at least 18 years of age and have a high school degree or GED

• Bachelor’s Degree preferred
• Experience with supporting individuals with disabilities preferred
• If driving for the agency, a valid driver’s license, proper registration, insurance and satisfactory driver’s record required- these credentials must be maintained for initial and continued employment
• Must submit to a criminal history review/ fingerprinting. All employment and continued employment are contingent upon receipt and maintenance of a satisfactory criminal records review/ fingerprint report.
• Drug Testing required upon hire
