FAMILY SUPPORT SPECIALIST (FSS)
Job description
The Family Support Specialist will provide intensive home visiting services to families. Home visits will be conducted using the FSU or GGK Curriculum, applying Reflective Strategies: The FSS will develop a Family Goal Plan with each parent. They will administer Perinatal Depression Screening (PHQ-9), Ages & Stages Questionnaire (ASQ/ ASQ-SE), and an approved Parent Child Interaction Tool, while providing regular documentation and data submission.
Responsibilities:- Initiates and maintains regular contact with families, primarily in their homes (some evenings and weekends as necessary).
- Establish a trusting relationship with families.
- Provide prenatal and other health education.
- Assist families to strengthen parent-child relationships using approved curriculum.
- Assist parents to improve their skills to optimize the home environment for the child’s well-being and safety.
- Conduct developmental screenings, and provide activities in the home to promote healthy child development; including referrals for EIP as needed.
- Assist families in making and attending health and human service appointments, including activities related to employment and educational goals.
- Participate in the coordination of special events and group activities and for parents and children.
- Participates in outreach, and other efforts to educate the community about the program.
- Attends all Hospital and HFNY trainings within the required timelines.
- Provides documentation of all above activities in accordance with Hospital and HFNY requirements and timelines.

Minimum of a high school diploma or equivalent, college coursework preferred.

No prior experience is required.

Prior experience working with or assisting children and families at risk in a community setting is preferred.

Successful parenting and/or child care experience preferred.
- Knowledge in infant and child development.
- Ability to be non-judgmental.
- Good problem-solving and practical skills.
- Demonstrates maturity and emotional well-being.
- Knowledge of community resources.
- Cultural sensitivity.
- Strong listening skills.
- Good writing and time management skills.
- Microsoft Word
- Bilingual a plus
- Must be available for evenings, some weekends, and occasional overnight travel.
- Ability to work in the field at least 3-4 days per week, conducting in-home assessments and attending community initiatives.
- Ability to fully utilize BHMC’s electronic record systems, equipment and other healthcare and billing systems relevant to this position.
- Position requires prolonged periods of standing, reaching, walking throughout the working day.
- Position will be required to stoop, bend, lift, and carry items weighing up to 25 pounds.
