Overview:
Position Overview
Overseen by the Clinic Management, the Fee For Service (FFS) Therapist is responsible for providing ongoing therapy, assessment and case management type services to individuals receiving services in the clinic.
Responsibilities:
Essential Job Functions
  • Effectively communicate and support the organization’s mission, vision and values
  • See individuals for routine appointments and initial assessments (as needed)
  • Ensure documentation meets quality and compliance thresholds as set by regulation and/or Agency guidelines
  • Utilize electronic medical record to complete required documentation as required by regulation and within mandated time frames
  • Prevent, recognize and report abuse toward the people we support
  • Assess and diagnose individuals as needed
  • Adhere to the organizations Code of Ethics
  • Always communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community
  • Complete all required trainings
  • Maintain confidentiality
  • Take part in new treatment initiatives including but not limited to, integrated care, smoking cessation treatment and co-occurring disorders treatment
  • Maintain and complete responsibilities of client caseload including but not limited, treatment plans, progress notes and assessments
  • Assist the Clinic Management in reporting activities including but not limited to Pscykes, OMH, Financial and County reporting requirements.
  • Act as part of an interdisciplinary team
  • Other duties as assigned
Qualifications:
Minimum Job Qualifications
  • Licensed therapist (LMHC, LMSW, LCSW) preferred, unlicensed and working towards licensure within the year accepted.
  • At least 2 years’ experience providing individual, group or family therapy to individuals with mental illness
  • Valid driver’s license and meets all the criteria to be a valid driver for the agency
  • If Licensed, a valid and active NPI, and registration with the Office of Professions

While performing the duties of this job, employees are regularly required to sit, walk and stand; talk or hear, both in person and by telephone; operate standard office equipment; reach with hands and arms; and lift up to 25 pounds
