JOB SUMMARY:
Under the direct supervision of the Public Health Advisor, the Fitness Coordinator (FC) is responsible for coordinating the Fitness Program at GLIIHC, including on-site supervision of the Fitness Center and communication with staff throughout the agency as well as contributing to fitness-related outreach activities. The Fitness Coordinator (FC) will serve to connect the community and staff to physical fitness and physical activity education opportunities. Through outreach and education, the FC will assist clients and patients in engaging in the recommended levels of physical fitness and strength training as medically allowable. In partnership with patients and their medical care team the FC will refer patients to internal programs or supports when appropriate and will also receive referrals from GLIIHC staff. This collaborative position will involve working with individuals across departments as well as organizations and individuals from throughout the community.

CERTIFICATE, LICENSES, REGISTRATIONS:
  • Preferred candidate will have a Fitness Certification by one of the following authorizing bodies: ACE, AFAA, ASCM, NSCA, NAFC, and/or NSPA.
  • Preferred candidate will possess an associate’s degree or its equivalent and a minimum of three years of related experience
  • CPR/AED and first aid trained and currently certification
QUALIFICATIONS:
  • Excellent oral/written communication, organizational and presentation skills
  • Good computer skills including Word, Excel, social media platforms
  • Preferred candidate will have experience designing and implementing individualized and group physical fitness programs
  • Experience working with youth through elders and medically complex patients effectively
  • Ability to support patients/clients working toward health and strength improvement under the recommendations of their primary care provider (PCP)
  • Ability to establish and maintain effective relationships with diverse groups of people
  • Self-motivated team member; able to work effectively without daily supervision
  • Preferred candidate will have access to a car, possess a valid Wisconsin driver’s license and adequate auto liability insurance to be able to travel locally and work irregular hours as needed
  • Familiarity with and adherence to evidence-based practices related to physical fitness, strength training, current or emerging recommendations
  • Experience with working in Native American communities or agencies a plus
ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Work with the Diabetes Program Coordinator to fulfill Special Diabetes Program for Indians (SDPI) requirements for individual and group physical fitness programming
  • Work closely with the Community Outreach and Health Promotion and Disease Prevention teams; collaborate with all in-house teams as requested
  • Lead group fitness activities, including working regularly with elders in the Work Out Low Fat Elders (WOLFE) Program and the Youth Empowerment Program (YEP)
  • Using tools from accredited fitness organizations, provide individual fitness assessments, consultations, and education to diverse clientele in a safe and respectful manner
  • Help users of the Fitness Center and fitness programming to develop self-efficacy regarding physical fitness/physical activity
  • Provide healthy activity outreach activities at on- and off-site events such as the Native Wellness Garden, the Community Health Fair, Mammogram Bus Days, other health-focused events
  • Give and receive referrals from staff, and document physical fitness activities where indicated in Epic (electronic medical records—training will be provided)
  • Coordinate a fitness calendar with assistance from the Marketing and Communications Director
  • Contribute regularly to written/online content for the agency, such as newsletters, blog posts, flyers, and other products promoting health and physical fitness
  • Attend meetings related to prevention topics on- and off-site
  • Represent GLIIHC in a professional way that promotes partnerships, engagement, enthusiasm for physical fitness activities as part of health maintenance and disease prevention
  • Regularly update skills and knowledge related to exercise as a prevention strategy for health and wellness
  • Work with Physical Therapy Department to assess Fitness Center/therapy needs and appropriate set-up of space
  • Provide ongoing assessment of Fitness Center needs including equipment, materials for self-guided workouts, ongoing training necessary, etc.
  • Ensure proper maintenance and repair of Fitness Center equipment and physical space (with support of Housekeeping and Security Team, outside repair persons as needed)
  • Follow GLIIHC’s protocols when client’s/patient’s need supersedes the FC’s scope of practice.
  • Develop culturally relevant outreach and education materials to support patient/client health
  • Participate in clinical and organization Quality Improvement initiatives on a regular basis
  • Organize and maintain program records and supplies, complete all paperwork in a timely manner
  • Travel on occasion for continuing education, training and conferences.
  • With supervisor support, set, follow, and report on quarterly goals to offer fitness activities to community and staff members
  • Other duties as assigned
OTHER SIGNIFICANT FACTORS:
The physical requirements described here are representative of those that must be met by an employee to successfully perform the essential functions of the job. While performing the duties of this job, the employee is regularly required to sit; use hands to manipulate objects, tools or controls; reach with hands and arms; and talk and hear. The employee must frequently lift and/or move Fitness Center equipment which may weigh in excess of 50 pounds, and must be able to support clients with weight training during workout sessions. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception and the ability to adjust focus. Noise level in the work environment is usually moderately loud.

Work is performed in a fitness center and/or office environment utilizing various physical fitness equipment and office equipment including, but not limited to, a computer, copy machine, fax machine, and multi-line phone. Ability to communicate effectively over the telephone, by computer and in person. Ability to perform well in a fast paced work environment.
