HouseCall MD is seeking a highly qualified and motivated Nurse Practitioner to join our team in West Covina, CA. In this role, you will be responsible for managing an adult patient panel, providing episodic, chronic, acute, counseling, preventive, and palliative care, with the collaboration of our medical director. This is a flexible, house call position that requires you to visit patients in their homes within the city.

We are looking for a nurse practitioner with an interest in geriatrics, family medicine, or prior home care experience. The ideal candidate will be compassionate towards patients with complex medical and social needs, have strong communication skills, and be comfortable with technology, including EHR documentation. Bilingual candidates are preferred.

HouseCall MD will provide all office support, administrative assistance, malpractice insurance, EHR, and patient scheduling. Requirements:

• Graduate of an accredited program
• Current DEA Registration Number
• Active California license
• Board Certified as a Family Nurse Practitioner or Geriatric Nurse Practitioner
• Palliative Nursing (CHPN, advanced) preferred
• National Provider Identification Number (NPI)
• Valid Driver's License with a clean driving record
