Minimum requirements: Valid Nurse Practitioner License in the Commonwealth of Kentucky, DEA License, Hospice and Palliative Care experience preferred; Must be able to pass a criminal background check, pre-employment, and random drug screen. Must have reliable transportation and appropriate auto insurance.
