Position Summary: The MDS Coordinator will provide support and assistance to the Director of Nursing with ensuring that all documentation in the center meets Federal, State, and Certification guidelines. The MDS Nurse Coordinator will coordinate the entire RAI process assuring the accuracy, timeliness, and completeness of the MDS, CAA’s, and Interdisciplinary Care Plan. The MDS Nurse Coordinator conducts the nursing process-Assessment, Planning, Implementation and Evaluation-under the state’s Nurse Practice Act for Registered Nurse or Practical Nurse Licensure.
Job Duties:
Assist the center in ensuring adherence to Federal and State regulations and certifications. Provides follow-up as necessary to the Director of Nursing and or Administrator of any noted findings that are in violation of Federal and State regulations or findings that could potentially have an adverse effect for the resident and or center in references to standards of care and services and or documentation standards.- Ensures that issues/concerns identified by the resident, family members and or responsible party during the assessment process, care conferences and or family meetings is communicated to the appropriate staff personnel, Director of Nursing and/or Administrator for follow-up as appropriate.
- Physically assesses each resident and completes interviews with resident, family, responsible party and center staff as necessary to assure the standard of practice is adhered to as instructed in the most recent version of the RAI/MDS manual.
- Maintains compliance to the timelines for completion of all required assessments as instructed in the most recent RAI/MDS Manual. Ensures that appropriate signatures are obtained as required for all completed assessments.
Graduate of an accredited School of Nursing.- Valid license as Registered Nurse or Licensed Practical Nurse in the State of practice.
- Minimum of one (1) year of nursing experience in a setting serving the same age/type of resident population served by this facility.
- Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required
- Knowledge of regulations and Minimum Data Set
- Knowledge of care planning process
- Computer literate
- Excellent communication and interpersonal skills.
- Excellent verbal, written, and interpersonal communication skills.
