Under the general supervision of the Health Home Director, the Health Home Coordinator provides support for designated health home clients which includes coordinating an array of services designed to improve the health of high needs, high risk DWIHN members.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
- Provides care coordination including assessment, care planning and monitoring of client’s status.
- Ensures implementation and coordination of services.
- Provides administrative support and coordination for all activities within the Health Home and Certified Community Behavioral Health Clinic (CCBHC) programs.
- Manages all enrollments into health homes in collaboration with provider partners utilizing the online Waiver Support Application (WSA) and Electronic Health Records.
- Processes detailed and accurate records for enrollment and disenrollment.
- Runs reports on a regular basis for program enrollment, disenrollment, claims and other reports as requested by the Health Home Director and other DWIHN team members.
- Provides support to the Health Home team for training, general supervision, oversight, as well as technical assistance across systems for Health Home and CCBHC initiatives.
- Assists Health Home Director and Health Home team with monitoring partner performance, enrollee records, and adherence with program requirements, goals and expectations.
- Monitors providers and analyzes data to make recommendations to the Health Home Director and Health Home team to improve the quality of our services and outcomes.
- Coordinates, schedules and documents in collaborative workgroups to address projects, issues and/or barriers.
- Collaborates with Lead Entity care management staff and Health Home Partners to identify a beneficiary’s optimal setting of care.
- Coordinates and utilizes Health Information Technology (HIT) to maximize care coordination and care management.
- Serves as a key point of contact for providers and partners around the Health Home and CCBHC programs.
- Ensures person-centered and integrated recovery action planning that coordinates and integrates all clinical and non-clinical health care related needs and services.
- Provides quality-driven, cost-effective health home services in a culturally competent manner that addresses health disparities and improves health literacy.
- Develops and maintains working relationships with primary and specialty care providers including Community Mental Health Service Providers and inpatient facilities.
- Collects and reports on data that permits an evaluation of increased coordination of care and chronic disease management.
- Assists with prevention and wellness initiatives and referral tracking.
- Provides training and technical assistance to provider partners.
- Stays informed and up to date on Health Home and CCBHC programs.
- Works as part of the interdisciplinary team to facilitate consultation, collaboration and care continuity across care settings for the best possible treatment.
- Performs related duties as assigned.

- Knowledge of Integrated Care Systems in relation to Health Homes, CCBHC and integrated care services.

REQUIRED EDUCATION:
A Bachelor’s Degree from a recognized college or university in the Human Services, the Social Services, Nursing, Public Health, Public Administration, Healthcare Administration, Health Management, Psychology, Counseling, Social Work or a related field.
REQUIRED EXPERIENCE:
Two (2) years of professional experience in a behavioral healthcare or mental health setting.
AND
One (1) year of professional experience working in community-based programs with agencies that provide services for persons with behavioral health disorders.
REQUIRED LICENSE(S).
A valid State of Michigan Driver’s License with a safe and acceptable driving record.
WORKING CONDITIONS:
Work is usually performed in an office setting but requires the employee to drive to different sites throughout Wayne County and the State of Michigan. This position can work remotely with supervisory approval. Currently this position is primarily a remote position.
